Abstract:
زمینه و هدف: تغذیه مناسب در تحول مغزی و شناختی، عاطفی، و اجتماعی کودکان نقش بسیار مهمی دارد. مشکلات خوردن در کودکان پیشدبستانی و دبستانی مسئلهای شایع بوده و درمان آن ضروری است. از این رو پژوهش حاضر با هدف مقایسه برنامه آموزش والدین و درمان رابطه والد و کودک بر کاهش مشکلات خوردن کودکان 3-6 سال انجام گرفت.
روش: این پژوهش از نوع شبه آزمایشی با دو گروه آزمایش و یک گروه گواه است. نمونه مورد مطالعه 75 نفر از مادران کودکان دارای مشکلات تغذیهای 3 تا 6 ساله شهر تبریز در سال 95 بودند که به شیوه نمونه گیری دردسترس انتخاب و به صورت تصادفی در سه گروه شامل 2 گروه آزمایش و 1 گروه گواه قرار جایدهی شدند. برای جمع آوری داده ها از پرسشنامه رفتار خوردن کودکان (وارل، گاتری، ساندرسون و راپورت، 2001) استفاده شد. مداخله آموزش مدیریت والدین در 6 جلسه 90 دقیقه ای و مداخله بهبود رابطه والد - کودک در 10 جلسه 2 ساعته انجام شد. پیگیری نتایج نیز یک ماه بعد از انجام مداخله ها انجام شد. برای تحلیل دادهها از روش تحلیل واریانس آمیخته با عامل درونگروهی (پیش آزمون، پس آزمون، و پیگیری) و بین گروهی (درمان رابطه والد کودک و آموزش مدیریت والدین) با نرم افزار spss 20 استفاده شد.
یافته ها: نتایج تحلیل داده ها نشان داد که بین سه گروه از نظر مشکلات خوردن تفاوت معنی دار وجود دارد. هر دو مداخله، مشکلات خوردن را کاهش دادند، اما برنامه آموزش والدین اثربخشی بیشتری در کاهش هر دو مؤلفه مشکلات خوردن یعنی گرایش به غذا (عامل درونگروهی، 0/001 = p، 818/84= F و بینگروهی 0/001 = p، 31/32= F ) و اجتناب از غذا (عامل درون گروهی 0/001 = p، 1237/56= F ، عامل بین گروهی 0/001 = p، 47/86= F) داشت.
نتیجه گیری: بر اساس یافته های این مطالعه می توان نتیجه گرفت که برنامه آموزش مدیریت والدین با افزایش دانش و مهارت والدگری والدین و آموزش راهکارهای رفتاری به آنها، رفتارهای معیوب بین مادر و کودک را اصلاح کرده و از این طریق به بهبود مشکلات خوردن منجر شده است. درمان رابطه والد -کودک نیز با ایجاد رابطه دلبستگی ایمن و برآوردن نیازهای هیجانی باعث بهبود مشکلات خوردن شده است.
Background and Purpose: Proper nutrition plays an important role in the brain and cognitive, emotional, and social development of children. Eating problems in preschool and primary children are common and need to be treated. Therefore, the purpose of this study was to compare of the effect of child-parent relationship therapy (CPRT) and parent management training (PMT) on nutritional problems in children 3 to 6 years
Method: This is a quasi-experimental study with two experimental and one control group. The sample consisted of 75 mothers of 3 to 6 year old children with nutritional problems in Tabriz in 2017 who were selected by convenience sampling and randomly assigned into three groups including 2 experimental and 1 control group. Child Eating Behavior Questionair (Warle, Guthrie, Sanderson, Rapport, 2001) was used for data collection. Parent management training was performed in six 90-minute sessions and child-parent relationship therapy in ten 2-hour sessions. Follow-up was also performed one month after the intervention. Data analysis was performed using mixed between-within subject ANOVA with SPSS 20 software
Results: The results of data analysis showed that there was a significant difference between the three groups in terms of eating problems. Both interventions reduced eating problems, but the parental training program had more efficacy in reducing both components of eating problems, namely, food intent(within group factor P=0.001, F=818.84 and between group factor P=0.001, F=31.32) and food avoidance (within group factor P=0.001, F=1237.56 and between group factor P=0.001, F=47.86).
Conclusion: Based on the findings of this study, it can be concluded that the parent management training program can improve the inappropriate behaviors between mother and child by improving the parenting skills and behavioral strategies. Child-parent relationship therapy has also improved eating disorders by establishing a secure attachment relationship and meeting emotional needs.
