Abstract:
اهداف: افسردگی اساسی دوّمین بیماری شایع دنیا و مهمّترین عامل تخریب عملکرد افراد و کاهش کیفیت زندگی آنان است. علایم باقیهای (مانند اندیشناکی) که پس از درمان به جا میمانند، احتمال بازگشت/ عود اختلال را بالا نگه میدارند. این پژوهش با هدف بررسی اثربخشی رفتار درمانی شناختی متمرکز بر اندیشناکی در کاهش میزان اندیشناکی، علایم افسردگی و بهبود راهبردهای حل مساله، انجام شد. مواد و روشها: طرح پژوهش شامل طرح تجربی تک آزمودنی با خطّ پایۀ چندگانه و اندازهگیری مکرّر است. هفت نفر از طریق نمونهگیری هدفمند، یک نفر به عنوان شرکتکنندۀ آزمایشی (زن) و شش نفر گروه نمونه (سه زن و سه مرد) در یازده الی بیست جلسۀ هفتگی 60 دقیقهای شرکت کردند. ابزارهای پژوهش شامل SCID، مقیاس سبک پاسخ (RSQ)، مقیاس افسردگی بک-ویراست دوّم و مقیاس حل مسالۀ کسیدی و لانگ بودند. برای سنجش عملیاتی میزان کارآیی نتایج پژوهش، ملاکهای اندازۀ تغییر (درصد بهبود و شاخص تغییر پایا)، کلیّت تغییر، عمومیت تغییر، میزان پذیرش، ایمنی و ثبات مورد بررسی قرار گرفتند. یافتهها: رفتار درمانی شناختی متمرکز بر اندیشناکی، موجب کاهش معنادار میزان اندیشناکی (اندیشناکی) اندیشناکی و علایم افسردگی در شرکتکنندگان شد. هم چنین، تغییرات نسبی در راهبردهای حل مسالۀ افراد مشاهده شدند. نتیجهگیری: رفتار درمانی شناختی متمرکز بر اندیشناکی، اثر مطلوبی در تضعیف عادت اندیشناکی و کاهش علایم افسردگی در افراد نشان داد. درمانگران میتوانند از رویکرد مذکور، به تنهایی یا به همراه سایر رویکردهای درمانی برای حل مشکل اندیشناکی استفاده کنند.
Objectives: Rumination is a common residual symptom of depression which plays an important role in onset and permanence of depressive episodes. The aim of this study is to evaluate the efficacy of Rumination-Focused cognitive behavior therapy on decreasing rumination habit, depressive symptoms and improving problem solving strategies.Methods: The research method was single-subject with multiple baseline and repetitive evaluation design. Seven participants (4 women and 3 men-one woman as pilot participant) meeting the including criteria, took part in 12, 60 minute therapy sessions. Depression, rumination and problem solving strategies were measured by BDI-II, Response Style Questionnaire (RSQ) and Cassidy and Long’s problem-solving Scale, before first session, during therapy sessions and after last one. Treatment specifically focused on switching patients from less helpful to more helpful styles of thinking through the use of functional analysis, experiential/imagery exercises and behavioral experiments. Results: Treatment produced significant improvements in depressive symptoms and rumination: Nearly % 71/43 of the sample showed % 50/6 reduction in RSQ and % 51/6 reduction in BDI-II. The improvement was sustained in one month fallow up. The sample’s scores in orienting strategy of Cassidy and Long’s problem-solving Scale showed some increasement. After treatment, participants did not fulfill criteria for Major Depressive Disorder.Conclusion: Treating depressive rumination appears to yield generalized improvement in depression and co-morbidity. This study provides preliminary evidence that rumination-focused CBT may be an efficacious treatment for medication-refractory residual depression.
