چکیده:
هدف : هدف پژوهش حاضر بررسی اثربخشی درمان شناختی- رفتاری گروهی در کاهش شدت درد و پریشانی های روانشناختی زنان مبتلا به کمردرد مزمن بود.
روش : در این پژوهش نیمهآزمایشی، از طرح پیشآزمون- پسآزمون با گروه کنترل استفاده شد؛ از میان تمامی زنان مبتلا به کمردرد مزمن مراجعهکننده به بیمارستان امام خمینی (ره) و یک مطب خصوصی در شهر تهران در سال 1389، تعداد 30 زن با میانگین سنی 33 سال و 7 ماه به طور تصادفی انتخاب و به تصادف در دو گروه آزمایش و کنترل جایگزین شدند. گروه آزمایش در 12 جلسه 90 دقیقهای درمان شناختی- رفتاری تورک و فری به صورت گروهی شرکت کرد و گروه کنترل تنها پی گیری شد. هر دوگروه پیش و پس از جلسه های درمانی به دو مقیاس ناتوانی کمردرد کبک ( QBPDS ) و افسردگی، اضطراب، استرس ( DASS ) پاسخ دادند. داده ها با آزمون t مستقل تحلیل شدند.
یافتهها : یافته ها نشان گر کاهش معنادار شدت کمردرد (p<0/05 ) و افسردگی، اضطراب و استرس (p<0/05 ) در گروه آزمایش به دنبال شرکت در جلسه های درمانی بود.
نتیجهگیری : درمان شناختی- رفتاری با تغییر رفتارهای ناکارآمد، تصحیح شناختهای ناسازگار و احساسهای مخرب مرتبط با درد، باعث میشود اثرهای زیانبار درد به کمترین حد برسد و پریشانی های روانشناختی بهبود یابد.
Objectives : The aim of this study was to investigate the effectiveness of group cognitive-behavior therapy on decreasing severity of pain and psychological distress among women with chronic back pain.
Method: This research is a semi-experimental study with pretest-posttest design and control group. Among females with chronic back pain referred to Imam Khomeini Hospital and a clinic in Tehran in 2010، 30 participants with mean age of 33 years and 7 months were selected and randomly assigned to two equal experimental and control groups. Experimental group during twelve 90-min sessions received Turk and Free cognitive-behavior therapy، and control group were just followed. All participants completed the items of Depression، Anxiety، and Stress Scale (DASS)، and Quebec Back Pain Disability Scale (QBPDS) prior to and after the intervention. The data were analyzed using independent t-test.
Results: The findings revealed a significant decrease of back pain (p<0.05)، and depression، anxiety and stress symptoms (p<0.05) in experimental group who participated in therapeutic sessions.
Conclusion: Cognitive-behavior therapy could minimize adverse effects of pain and decrease psychological distress by changing dysfunctional behaviors، correcting counterproductive cognitions and deteriorating sensations related to pain.
