چکیده:
سابقه و هدف: علیرغم اینکه آسیب مفصل، جنبه های مختلف حس عمقی از قبیل وضعیت و تعادل را تحت تاثیر قرار می دهد و از سوی دیگر، نقش توانبخشی بر بهبود عملکرد و تحرک افراد در ضایعات مختلف ثابت گردیده است، ولی اهمیت برنامه های توانبخشی در بهبود حس عمقی مورد سوال است. هدف از این تحقیق، تعیین تاثیر تمرینات ثبات دهنده کمر در بهبود حس عمقی و همچنین بر امتداد ستون فقرات لومبو ساکرال در افراد سالم و جوان بود.
مواد و روش ها: در این مطالعه کارآزمایی بالینی، 30 زن جوان سالم وارد مطالعه شدند که به طور تصادفی در دو گروه آزمون (20 نفر) و کنترل (10 نفر) قرار گرفتند. از آنجا که تغییر وضعیت لگن بر وضعیت کمر اثر می گذارد، از ارزیابی حس عمقی لگن به عنوان معیاری برای بررسی حس عمقی لومبوساکرال استفاده شد. خطای بازسازی زاویه هدف برای ارزیابی حس عمقی لگن توسط شیب سنج لگنی اندازه گیری شد؛ همچنین مقدار لوردوز کمر و تیلت استراحت لگن قبل و بعد ازتمرینات برای بررسی تاثیر احتمالی این تمرینات بر امتداد ناحیه لومبوساکرال اندازه گیری شد. در گروه آزمون، افراد در ده جلسه، هرروز ورزش های ثبات دهنده کمر را انجام دادند. گروه کنترل در طی این ده روز هیچ ورزشی انجام ندادند. در هر دو گروه قبل و بعد از ده جلسه خطای حس عمقی با آزمون های غیرپارامتری مورد قضاوت آماری قرار گرفت.
یافته ها: در گروه آزمون، خطای مطلق حس عمقی از مقدار 2/1±4/1 در جلسه اول، به 1/0±006/0 درجلسه دهم کاهش معنی دار پیدا کرد (0001/0p<)؛ در حالی که در گروه کنترل، پس از ده جلسه خطای مطلق حس عمقی تغییر معنی داری پیدا نکرد (3/0=p). مقدار خطای مطلق جلسه دهم بین دو گروه تفاوت معنی دار داشت (0001/0p<). همچنین بین میزان اولیه خطای عمقی با میزان تغییر آن ارتباط آماری معنی دار وجود داشت (99/0=p). در هر دو گروه، پس از ده جلسه مقدار لوردوز و تیلت استراحت تغییر آماری معنی داری پیدا نکرد.
نتیجه گیری: ده جلسه تمرین ثبات دهنده کمر می تواند کاهش معنی دار خطای حس عمقی ناحیه لومبو ساکرال را در افراد سالم و جوان ایجاد کند. میزان این تغییر با مقادیر اولیه خطای حس عمقی ارتباط دارد؛ یعنی افرادی که خطای اولیه بیشتری دارند، در اثر تمرین بهبودی بیشتری پیدا می کنند.
Background and Aim: Despite the fact that joint injury influences different aspects of the propriocepion such as posture and balance، and despite the role of rehabilitation programs in increasing the function and activity، the importance of these programs on proprioception has been remained under question. The aim of this study was evaluating the effect of lumbar stabilization exercises on the lumbosacral proprioception in healthy young women. Methodes and Materials: In this clinical trial study، 30 healthy young women were randomly assigned in two exprimental (n=20) and control groups (n=10). Since the change in pelvic position affects on the lumbar spine position، evaluation of the pelvic proprioception was considered as an scale to evaluate the lumbosacral proprioception. Target angle reproducing error for evaluation of the pelvic proprioception was measured by pelvic inclinometer. The lumbar lordosis and pelvic tilt in rest position were measured to examine the probable effects of the exercises on the alignment of the pelvis and lumbar spine. Subjects in the exprimental group attended in ten exercise sessions and subjects in control group did not do any exercises. Proprioception error was measured before and after ten sessions in both groups and analysed by nonparametric tests. Results: In the exprimental group، the absolute proprioception error was reduced statisticaly (P<0.0001) from the 1.4±1.2 in first session to 0.006±0.1 in last session but in the control group the absolute proprioception error did not change significantly. The absolute error was different significantly (P<0.0001) between the two groups after ten sessions. Also there was excelent relationship between the proprioception error at the first session and its changes (R=0.99). The amount of lumbar lordosis and pelvic tilt angle did not change in none of the groups after the sessions. Conclusion: Ten sessions of lumbar stabilization exercises reduces the lumbosacral proprioception error in young healthy women. The amount of this reduction had significant relationship with proprioception error at the first session; that is the subjects who had more innitial error gained more improvement through exercises.
