چکیده:
سابقه و هدف: رواسکولاریزاسیون سریع، هدف اصلی درمان بیماران با انفارکتوس حاد میوکارد (MI) است که طی چندین سال گذشته با روش P-PCI (Primary- Percutaneous Coronary Intervention) انجام می گیرد. با توجه به این که ترومبولیتیک تراپی در بسیاری از مراکز کشور به دلیل عدم امکان آنژیوپلاستی انجام می گیرد، هدف از این مطالعه بررسی نتایج این دو روش، در مراکز دانشگاهی کشور است.
مواد و روش ها: تحقیق به روش همگروهی انجام گرفت. تشخیص بیماری با روش اســتاندارد انجام شد و ترومبولیتیک تراپی (TT) و P-PCI طبق روشهای استاندارد انجام شد. بیماران دو گروه همسان سازی شدند و بعد از درمان، در مدت بستری پیگیری شدند و تاثیر روش های درمانی با بررسی میزان Ejection Fraction (EF) و ST رزولوشن بیش از 50% و میزان عوارض در دو گروه مقایسه گردید. محل انفارکتوس میوکارد با نتیجه درمان در دو گروه، مقایسه شد.
یافته ها: تحقیق در 287 بیمار شامل 144 بیمار در گروه TT و 143 نفر در گروه P-PCI انجام شد. میزان EF در گروه TT 9/11±8/43 و در گروه P-PCI 8/9±1/42 (2/0p<) و ST رزولوشن بیش از 50% در گروه TT برابر7/66% و در گروه P-PCI برابر 6/88% بود (02/0p<). میزان عوارض در دو گروه مشابه بود. نتایج بر اساس محل MI نیز مشابه بود.
نتیجه گیری: به نظر می رسد که P-PCI موثرتر از TT در درمان بیماران مبتلا به ST Elevation Myocardial Infarction (STEMI) باشد و بنابراین بررسی یک تحقیق کارآزمایی بالینی توصیه می شود.
Background & Aim: Early revascularization is the main step in treatment of patients with acute MI which since a few years ago has been done via P-PCI. Since thrombolytic therapy is performed as a standard method of therapy of acute MI in all centers not equipped with angiography and angioplasty systems، we decided to compare these two methods in the university centers in Iran.
Materials & Methods: This prospective study was conducted on STEMI (ST Elevation MI) patients referred to emergency department of Shohadaye Tajrish and Shahid Modarres Hospitals، in Tehran، for early revascularization using thrombolytic therapy or Primary PCI. We compared these two methods of revascularization based on the effects on ejection fraction، ST-resolution and complications.
Results: 287 patients (144 in T.T group and 143 in P-PCI group) were enrolled in our study. There were no differences in ejection fraction between the two groups، but ST-resolution was significantly better in the P-PCI group (P< 0.001). The rate of complications was similar in both groups.
Conclusion: It seems the P-PCI is more effective than T.T in patients with STEMI.
