چکیده:
زمینه و هدف
علیرغم تلاش بیمارستانها، سرعت کاهش اشتباهات پزشکی هنوز هم آهسته است. بسیاری از خطاها اتفاق میافتد و هزاران بیمار و پزشک در اثر خطاهای پزشکی رنج میبرند. اعضای سازمانهای بهداشتی و درمانی، عموما برای افشای خطاهای پزشکی و عذرخواهی از فرد آسیبدیده، آموزشی نمیبینند و ممکن است از افشای خطا و عذرخواهی خودداری کنند. هدف این پژوهش، واکاوی پدیده عذرخواهی پزشکان متعاقب وقوع خطاهای پزشکی است.
مواد و روش ها
روش پژوهش، کیفی (مبتنی بر مصاحبه) با رویکرد تحلیل محتوای قراردادی است. دادهها با انتخاب هدفمند ۲۵ نفر از پزشکان و دستیاران ارشد شاغل در بیمارستانهای شهید محمدی، کودکان و خلیج فارس بندرعباس از طریق مصاحبههای نیمهساختاریافته گردآوری شد. استراتژی تحلیل دادهها، تحلیل مضمون است بدینمنظور از نرمافزار MAXQDA ۱۰ کمک گرفته شد. بازه زمانی پژوهش سال ۹۸-۱۳۹۷ است.
یافته ها
در این پژوهش سه مضمون فراگیر (پیشایندها، پدیده عذرخواهی پزشکان و پیامدها)، هشت مضمون سازماندهنده (هنجارهای جامعه پزشکی، عوامل اجتماعی، عوامل فردی، اجزای عذرخواهی، شرایط عذرخواهی، کسب مقبولیت اجتماعی، فشارهای درونی و بیرونی و تسهیل شرایط درمان برای بیمار و پزشک) و ۲۷ مضمون پایه شناسایی شد.
ملاحظات اخلاقی
برای انجام پژوهش کد اخلاق با شناسه ir.hums.rec.۱۳۹۷.۲۳۷ از کمیته اخلاق دریافت شد. برای داوطلبانهبودن شرکت در پژوهش ابتدا فرم رضایتنامه کتبی به افراد داده شد و پس از اعلام رضایت، اجازه برای مصاحبه و یادداشتبرداری کسب شد. در زمینه محرمانهبودن اطاعات شخصی به آنها اطمینان داده شد.
نتیجه گیری
ترس از تبعات قانونی و از دستدادن اعتبار باعث میشود که پزشکان را در ارایه عذرخواهی محتاطتر، اما مولفههای آموزش، فرهنگ و ویژگی شخصیتی، ارایه و پذیرش عذرخواهی را تسهیل میکنند. از سویی دیگر، انجام عذرخواهی روابط بیمار و پزشک را بهبود داده و از بروز مشکلاتی مانند فشارهای روحی و روانی و پیگیری قانونی جلوگیری میکند.
Background and Aim
Nurses have a very important and prominent role in the health care system, and turnover of nurses in health care system, disrupts the functioning of the organization. There are many factors that can be associated with the turnover of nursing services. The purpose of this study was to investigate the relationship between moral distress and Nurses' Turnover Intention in intensive care unit nurses.
Materials and Methods
This study is a descriptive-correlational study, which was performed using census sampling on 117 nurses working in the intensive care units of Qom hospitals in 2016. Data were collected using the Hinshaw Anticipated Turnover Scale, and the Jamton 30- Questionnaire. Data were analyzed by SPSS software 16 and descriptive statistics and statistical tests of multivariate logistic regression.
Findings
The results showed that there was a significant relationship between moral distress and the Nurses' Turnover Intention in intensive care units nurses (p=0.03, OR: 2.44). The mean score of moral distress in the participants in the study was 137.53 with a standard deviation of 23.14. Also, the mean score of Nurses' Turnover Intention was 3.44 with a standard deviation of 0.948. Ethical Considerations: This article is the result of an approved research project at Qum University of Medical Science. During the research process, the university's ethical policies were complied with in accordance with the principles of the Helsinki Declaration, including obtaining informed consent from participant.
Conclusion
According to the results of this study, it is necessary for nurses to pay more attention to moral distress and Nurses' Turnover Intention. Therefore, measures should be taken to reduce ethical challenges, moral distress, and subsequently Nurses' Turnover Intention in the clinical setting.
