چکیده:
نظر به اهمیت مراقبتهای دوران بارداری، حفظ سلامت مادر و تولد نوزاد سالم و به منظور تعیین نقش زایمانهای آنانسفال در بروز عواقب حاملگی این تحقیق در سال 1372 انجام گرفته است. این تحقیق به روش تحلیلی روی 35 مادر با زایمان آنانسفال و 43 نفر با زایمان طبیعی انجام گرفت. افراد مورد در یک بررسی روی 149000 زایمان در 6 مرکز آموزشی شهر تهران شناسایی و گروه شاهد آنها از لحاظ هم زمانی زایمان، سن، وضعیت اقتصادی-اجتماعی و محل سکونت کاملا با افراد گروه مشابه سازی شده اند. نتیجه حاملگی گروه مورد و شاهد شامل پارگی ممبرانها، طول حاملگی، نمایش جنین، هیدرآمنیوس، مرگ جنین، Sex Ratio و آنومالیها از روی پرونده مادران و نوزادان آنها بررسی و ثبت گردید. حاملگی آنانسفال خطر نسبی ابتلا زایمان (Odds Ratio: OR) به پارگی ممبرانها را بیش از 7 برابر افزایش می دهد (P<0.01) و موجب افزایش زایمان قبل از ترم به میزان 6 برابر بیشتر از گروه شاهد می گردد (P<0.0005) و نمایش جنین از نوع غیر سفالیک را 46 درصد، (P<0.0005) افزایش می دهد. Sex Ratio در گروه مورد 0.52 و در گروه شاهد 1.7 می باشد (P<0.025). همچنین هیدرآمنیوس در 90 درصد گروه مورد ایجاد شده و بالاخره خطر نسبی ابتلا به ناهنجاریها را تا 19 برابر افزایش می دهد (P<0.0005). با توجه به نقش مهم زایمانهای آنانسفال در بروز عواقب ناخوشایند حاملگی، توصیه می گردد که با بروز مجموعه ای از این عوامل، احتمال وجود زایمان آنانسفال نیز مدنظر قرار گیرد.
No doubt the well being and health conditions of a pregnant women and consequently her progeny has been a prime importance for human beings. Like wise the complications during this rather vital period of humans’ life has been a matter of great concern. In 1991 the idea of this research started to study the role of anencephaly as a causation of complicated pregnancies. Thirty five mothers with anencephaly and 43 mothers with normal delivery were included in this analytical research. The cases were recognized in a survey which has been done on 149000 births in 6 educational centers of Tehran and their control group matched completely for the same time of birth, age, socio-economic status and living environment. Pregnancy outcome of control and study groups which were all in the records as membrane rupture, pregnancy length, fetal presentation, hydramnios, fetal death, sex ratio anomalies were studied. Anencephaly increased odds ratio of membrane rupture more than 7 times (P<0.001), preterm birth, compared to control group was more than 6 times (P<0.005) and acephalic fetal presentation 46% (P<0.0005). In the study group, sex ratio was 0.52 and in control was 1.7 (P<0.025). Also, hydramnios had appeared in 90% of study group and finally, odds ratio to abnormalities showed an increase up to to 19 times (P<0.0005). Considering the important role of anencephaly in pregnancy complications, it is recommended that with the appearance of a set of these factors, the probability of anencephalic pregnancy should be taken serious
