چکیده:
مقدمه: بیماری قلبی مادرزادی شایعترین بیماری دوران جنینی و دومین علت مرگ در نوزادان و کودکان است. هدف مطالعه حاضر تشخیص عوامل کلیدی روانشناختی و زیستی تاثیرگذار بر پیامدهای روانشناختی ین کودکان بود.
روش کار: این مطالعه از نوع همبستگی بود. جامعه آماری شامل تمام کودکان 12-8 سال مبتلا به بیماری قلبی مادرزادی از نوع سیانوز مراجعهکننده به بیمارستان قلب شهید رجایی بود. 338 نفر از کودکان و والدینشان به شیوه نمونهگیری غیر تصادفی و از نوع در دسترس وارد مطالعه شدند. برای گردآوری دادهها از آزمون برج لندن Shallice (1982) که روی کودکان اجرا شد و پرسشنامههای نشخوار فکری Nolen-Hoeksema (1999) و فهرست اختلالهای رفتاری Rutter (1967) که توسط مادران پاسخ داده شد، استفاده گردید. به علاوه مادران به دو سوال ادراک مادر از شدت بیماری و شدت محدودیت فیزیکی کودک پاسخ دادند. اطلاعات مربوط به بیماری نیز از پروندههای کودکان استخراج شد. تجزیه و تحلیل دادهها در نرمافزار SPSS-25و با استفاده از روش تحلیل رگرسیون چندگانه به شیوه گام به گام انجام شد.
یافتهها: نشخوار فکری مادر، ادراک مادر و شدت محدودیت فیزیکی در مجموع 16 درصد از اضطراب/افسردگی کودک را پیشبینی کردند. نشخوار فکری مادر و حل مسئله 13 درصد از متغیر پرخاشگری/بیشفعالی را پیشبینی کردند. نشخوار فکری و حل مسئله 10 درصد از رفتار ضد اجتماعی و 7 درصد از ناسازگاریهای اجتماعی کودک را پیشبینی کردند. نشخوار فکری 8 درصد از نقص توجه و در نهایت دفعات بستری 2 درصد از حل مسئله را پیشبینی کردند. از بین این متغیرها، نشخوار فکری مادر بیشترین قدرت پیشبینی و تعداد دفعات بستری در بیمارستان کمترین قدرت پیشبینی را داشتند.
نتیجهگیری: عوامل روانشناختی به ویژه عوامل مرتبط با مادر، نسبت به عوامل مرتبط با جراحی نقش مهمتری در پیامدهای روانشناختی کودکان ایفا کردند.
Introduction: Tis study aimed to identify the key psychological and biological factors affecting the psychological outcomes of children with congenital heart disease.
Methods: A total of 338 children aged 8-12 years with congenital cyanotic heart disease were selected from patients who referred to the echocardiography section of the Rajaei cardiovascular hospital for a cardio checkup from October 2018 to September 2019. Then the Shallice (1982) Tower of London test, the Nolen-Hoeksema Rumination Response Scale (1999), and the Rutter (1967) Behavioral Scale were responded. The mothers of these children also answered two questions about the motherchr('39')s perception of the severity of the illness and the severity of the childchr('39')s physical limitations. Disease information was extracted from childrenchr('39')s records. Multiple regression analysis was used to analyze the data.
Results: Maternal rumination, maternal perception of severity of disease, severity of physical limitations, problem solving, number of surgeries, and hospitalizations were included as predictors in the analysis that could account for 2 to 16% of the variance in psychological outcomes. Among these variables, maternal rumination of depression symptoms had the highest and the number of hospitalizations had the least predictive power.
Conclusion: Psychological factors, especially maternal factors, played more important role in the psychological outcomes of children than the factors related to surgery. Therefore, providing psychosocial support and interventions is important especially for mothers who are primary caregivers.
