چکیده:
اهداف: اختلال طیف فلج مغزی شایعترین ناتوانی حرکتی دوران کودکی و یک وضعیت رشدی –عصبی است که در مراحل اولیه زندگی شروع شده و در تمام طول زندگی ادامه دارد. علاوه بر مشکلات حرکتی که بر میزان مشارکت کودک دارای اختلال طیف فلج مغزی تأثیر گذار است، محیط نیز می تواند میزان این مشارکت را کاهش یا افزایش دهد. هدف این مطالعه بررسی رابطه و نقش پیش بینی کننده عوامل محیطی بر میزان مشارکت کودکان دارای اختلال طیف فلج مغزی دایپلژی اسپاستیک بود.
روش بررسی: در این پژوهش مقطعی، جامعه مورد مطالعه کودکان دارای اختلال طیف فلج مغزی و والدین آنان بودند که از میان آنها، 116 کودک دایپلژی اسپاستیک و والدین آنها به روش نمونه گیری در دسترس در سالهای 1397-1396 در کلینیکهای کاردرمانی استان های تهران، مازندران و البرز انتخاب شدند. از والدین خواسته شد پرسشنامه اروپایی محیط کودک و پرسشنامه عادات زندگی که به ترتیب به منظور سنجش عوامل محیطی و مشارکت کودکشان بود را تکمیل نمایند. از سیستم طبقه بندی عملکرد حرکتی درشت برای سطح بندی عملکرد حرکتی درشت و توانایی راه رفتن و از مقیاس سطح شناختی اسپارکل، برای تخمین سطح هوشی استفاده گردید. از ضریب همبستگی پیرسون و رگرسیون خطی نرم افزار اس پی اس اس نسخه 22 به منظور تحلیل نتایج استفاده شد.
نتایج: 3/23درصد از این کودکان در سطح یک، 6/27 درصد در سطح دو، 9/31 در سطح سه و 2/17درصد در سطح 4 سیستم طبقه بندی حرکات درشت قرار داشتند. 7/89 درصد کودکان دارای سطح هوشی بالای هفتاد و پنج و 3/85 درصد این کودکان فاقد تشنج بودند. تمامی بخشها و خرده مقیاسهای عوامل محیطی با مشارکت همبستگی معناداری داشتند (01/0> P). بخش محیط فیزیکی دارای بالاترین همبستگی با مشارکت بود (01/0> P ، 811/0-=r). در مدل رگرسیونی خطی بخش محیط فیزیکی(01/0>P، 475/0-=B) و نگرش (05/0>P، 285/0-=B) در پیش بینی مشارکت نقش داشتند(71/0=R2).
نتیجه گیری: محیط فیزیکی علاوه بر همبستگی خوب با مشارکت اجتماعی در کودکان دارای اختلال طیف فلج مغزی دایپلژی اسپاستیک به همراه نگرش، جزو عوامل پیش بینی کننده بر میزان مشارکت نیز هستند. لذا با انجام تطابقات محیطی در خانه، جامعه، مدرسه و خصوصا حمل و نقل می توان بر میزان مشارکت کودکان دایپلژی اسپاستیک افزود.
Introduction: Cerebral palsy spectrum disorder is the most common motor disability in childhood and it is a neurodevelopmental condition beginning early in life and persisting throughout the lifespan. In addition to motor problems that affect the childchr('39')s participation, the environment factors might also reduce or increase the level of participation. The aim of this study was to The investigation of relationship and predictive role of environmental factors on participation of children with cerebral palsy spectrum disorder.
Methods and analysis: In this cross-sectional research, the study population was children with cerebral palsy and their parents. 116 children with diplegic spastic and their parents were recruited by convenience sampling method in 2018-2019 in occupational therapy clinics in Tehran, Mazandaran and Alborz provinces. Parents complete the European Children Environmental Questionnaire and the Life Habits Questionnaire, which were designed to assess the child environmental factors and participation, respectively.
The Gross Motor Function Classification System expanded and Revised and Sparkle cognitive level scale were used to classification of gross motor function especially walking and estimate intelligence quotient respectively. Pearson correlation coefficient and linear regression of SPSS software version 22 were used to analyze the results.
Findings: 23.3% of these children were in level I, 27.6% in level II, 31.9% in level III and 17.2% in level IV of the GMFCSE&R. 89.7% had an IQ above seventy-five and 85.3% had no seizures.
All parts and subscales of environmental factors had significantly correlated with participation (P 2 = 0.71).
Conclusion: Physical environment in addition to good correlation with social participation in children with diplegic spastic cerebral palsy spectrum disorder, along with attitude are predicting factors the level of participation. Therefore, modification and adaptations at home, community, school and especially transportation can increase the participation of children with spastic diplegic.
