چکیده:
مقدمه: اختلال دوقطبی نوع یک شدیدترین و شایعترین زیرگروه اختلال دوقطبی است. مطالعه بر چگونگی تاثیر علائم این اختلال بر روند بهبودی، میتواند در زمینه ی درمان و جلوگیری از عود به کار رود. بدین منظور هدف پژوهش حاضر مدل یابی علی بهبودی از اختلال دوقطبی نوع یک بر اساس آشفتگی خواب و سوگیری حافظهای مرتبط با مانیا، با نقش میانجی حافظه کاری میباشد. روش: این مطالعه ی مقطعی با روش نمونهگیری در دسترس روی 126 بیمار مبتلا به اختلال دوقطبی نوع یک در فاز شیدایی که به مطبهای روانپزشکی مراجعه کرده بودند، انجام شد. افراد نمونه 30 روز پس از پر کردن پرسشنامهها و سنجش اولیه، مورد سنجش مجدد شدت مانیا قرار گرفتند. دادهها با مدل معادلات ساختاری و با نرم افزار Smartpls-3 تحلیل شدند. یافتهها: نتایج نشان داد که عوامل آشفتگی خواب و حافظه کاری از عوامل موثر بر بهبودی از اختلال هستند (96/1 P)، ولی رابطه ی سوگیری حافظهای مرتبط با مانیا بر بهبودی از اختلال رد شد (96/1 >t و 05/0 > P). نتیجهگیری: آشفتگی خواب به صورت مستقیم و غیر مستقیم (از طریق تاثیر بر حافظه ی کاری)، و سوگیری حافظهای مرتبط با مانیا نیز از طریق تاثیر بر حافظه ی کاری میتوانند بر بهبودی از اختلال دوقطبی تاثیرگذار باشند.
Introduction: Bipolar disorder type 1 is the most severe and common subtype of bipolar disorder. A study of how the symptoms of this disorder affect the recovery process can be used in the treatment and prevention of recurrence. Therefore, the aim of this study was Causal modeling of recovery from bipolar disorder based on sleep disturbance and mania-related memory bias, with the mediating role of working memory.Method: This cross-sectional study was performed by available sampling method on 126 patients with type 1 bipolar disorder in the manic phase who were referred to psychiatric offices. Sample subjects were reassessed for mania severity 30 days after completing the questionnaires and initial assessment. Data were analyzed using structural equation modeling and Smartpls-3 software. Results: The results showed that sleep disturbances and working memory are effective factors in recovery from the disorder (t t and P> 0.05).Conclusion: Sleep disturbances can directly or indirectly (by affecting working memory) and mania-related memory bias can also affect the recovery from bipolar disorder by affecting working memory.Keywords: bipolar I disorder, sleep disturbance, mania-related memory bias, working memory
