اهداف: اختلال بلع یک مشکل متداول در بیماران دارای مالتیپل اسکلروزیس است. این اختلال متعاقباً میتواند باعث بروز مشکلات جدی از جمله افزایش خطر دهیدراسیون، کاهش وزن و پنومونیا مرتبط با آسپیراسیون شود که معمولاً منجر به مرگ در مراحل نهایی بیماری میشود. وجود یک ابزار غربالگری مؤثر، به تشخیص زودهنگام اختلال بلع کمک میکند و از بروز چنین مشکلاتی پیشگیری میشود. پرسشنامه اختلال بلع در مالتیپل اسکلروزیس، تنها پرسشنامهای است که به طور خاص برای این بیماران طراحی شده است. هدف از مطالعه حاضر، ترجمه و انطباق فرهنگی و بررسی روایی و پایایی این پرسشنامه در زبان فارسی و همچنین، بررسی فراوانی و شدت اختلال بلع بیماران مراجعهکننده به انجمن مالتیپل اسکلروزیس تهران و ارتباط آن با نوع بیماری، مدت زمان بیماری و میزان ناتوانی است.
روش بررسی: این پژوهش از نوع روششناسی و توصیفی ـ تحلیلی است که به روش مقطعی انجام شد. جامعه آماری پژوهش، بیماران دارای مالتیپل اسکلروزیس مراجعهکننده به انجمن مالتیپل اسکلروزیس تهران در سال 1397 بود و از میان آنها نمونه آماری، شامل 108 نفر به روش دردسترس انتخاب شدند. ابزارهای پژوهش شامل پرسشنامه دهآیتمی اختلال بلع در مالتیپل اسکلروزیس (در بخش اول مطالعه، ترجمه و ویژگیهای روانسنجی آن بررسی شد)، شاخص معلولیت اختلال بلع و پرسشنامه اطلاعات جمعیتشناختی بودند. در بخش آزمونسازی، پس از ترجمه و انطباق فرهنگی پرسشنامه، از تحلیل عاملی تأییدی برای بررسی روایی سازه و نیکویی برازش مدل ارائهشده، استفاده شد. برای بررسی روایی همگرا، ارتباط بین نمرات نسخه فارسی پرسشنامه با نمرات شاخص معلولیت اختلال بلع آزمون شد. همگنی آیتمهای پرسشنامه برای اندازهگیری سازه هدف و ثبات درونی آن بررسی شد. همچنین از روش آزمون ـ بازآزمون برای 19 بیمار تحت مطالعه برای ارزیابی پایایی استفاده شد. درنهایت، فراوانی و شدت اختلال بلع در بیماران مالتیپل اسکلروزیس و ارتباط آن با نوع بیماری، مدتزمان بیماری و میزان ناتوانی بررسی شد. برای تحلیل دادهها، از آزمونهای ضریب پایایی کودر ریچاردسون، ضریب همبستگی درونخوشهای، ماتریکس همبستگی بین آیتم، ضریب همبستگی اسپیرمن و آزمون منویتنی استفاده شد.
یافتهها: بر اساس یافتههای همبستگی بین آیتم، همبستگی مثبت بین همه آیتمها به جز آیتم ده به دست آمد. آیتم ده به دلیل همبستگی پایین با سایر آیتمها از پرسشنامه حذف شد. یافتههای تحلیل عاملی تأییدی، ساختار دوعاملی پرسشنامه شامل «اختلال بلع برای مواد غذایی جامد» و «اختلال بلع در مایعات» را تأیید کرد و نشان داد آیتم ده حداقل تأثیر را در اندازهگیری «اختلال بلع جامدات» دارد و با حذف این آیتم ثبات درونی پرسشنامه افزایش یافت. همچنین مدل نُهآیتمی تعدیلشده پرسشنامه، بهترین نیکویی برازش را با دادههای مشاهدهشده نشان داد. یافتههای روایی همگرا، وجود همبستگی مثبت و قابل ملاحظه بین نمرات پرسشنامه اختلال بلع و شاخص معلولیت اختلال بلع را نشان داد (0/693=Ps). ضریب پایایی کودر ریچاردسون، 80/856 به دست آمد که نشاندهنده ثبات درونی نسخه فارسی پرسشنامه است و پایایی آزمون ـ بازآزمون نیز 965/0 به دست آمد. نتایج مطالعه همچنین نشان داد 65/7 درصد از بیماران دارای اختلال بلع هستند که از نظر شدت، 9/25 درصد اختلال بلع خفیف و 39/8 درصد اختلال بلع هشداردهنده داشتند. اختلال بلع به طور معناداری با مدت زمان بیماری (0/198=P=0/040 rs) و میزان ناتوانی (0/235=P=0/014 rs) ارتباط داشت و میانگین نمرات پرسشنامه برای بیماران با نوع پیشرونده ثانویه بیماری نسبت به سایر زیرگروهها به طور معناداری بالاتر بود.
نتیجهگیری: نسخه فارسی تعدیلشده نهآیتمی پرسشنامه اختلال بلع در مالتیپل اسکلروزیس یک ابزار ساده، باثبات، روا و پایا برای تشخیص اولیه اختلال بلع و ارجاع زودهنگام برای ارزیابیهای ابزاری خاص است. با توجه به فراوانی بالای اختلال بلع در بیماران دارای مالتیپل اسکلروزیس، حائز اهمیت است که درمانگران ضمن نظارت دقیق بر بیماران حتی در مراحل اولیه بیماری به منظور تشخیص زودهنگام و طراحی برنامه توانبخشی، از بروز مشکلات جدی حاصل از این اختلال پیشگیری کنند.
Objective: Dysphagia is commonly encountered in Multiple Sclerosis (MS). It can consequently cause severe complications such as the increased risk of dehydration, malnutrition and aspiration pneumonia that associated with an increased risk of death in the late stages of the disease. These complications can be prevented with an effective screening protocol. The Dysphagia in Multiple Sclerosis (DYMUS) is the only questionnaire developed specifically for patients with MS. The aim of the present study was translation and cross-culturally adaptation of DYMUS for the Persian population and estimation of the questionnairechr('39')s reliability and validity. Also, we investigated the frequency and severity of dysphagia in the patients referring to the Tehran MS Society and its association with the course of MS, disease duration, and Expanded Disability Status Scale (EDSS).
Materials & Methods: This is a methodological and descriptive-analytic study. The statistical populations consist of the patients with MS referring to the Tehran Multiple Sclerosis Society in 2018. The study sample consisted of 108 patients who were selected through a convenient sampling method. The research instruments consisted of 10-items DYMUS questionnaire (in the first part of the study, translated and its psychometric properties examined), Dysphagia Handicap Index and a demographic information questionnaire. In methodological section, translation and cross-cultural adaptation were fulfilled and the Confirmatory Factor Analysis (CFA) was used to assess the construct validity and to test the goodness-of-fit of the presented model. The association between the DYMUS and Dysphagia Handicap Index (DHI) scores investigated to convergent validity. ‏ The inter-item correlation matrix was calculated to assess the homogeneity of the questionnaire items. Kuder - Richardson (KR-20) was measured to assess the internal consistency of the questionnaire. Also, the test-retest procedure was conducted on the 19 subjects understudy to assess the reliability. Finally, the frequency and severity of dysphagia and its association with the course of MS, disease duration, and EDSS was investigated. KR-20 reliability coefficient, Intraclass Correlation Coefficient (ICC), inter-item correlation matrix, Mann-Whitney U test and rank correlation coefficient or Spearmanchr('39')s were used to inferential statistical analysis.
Results: Analysis of the inter-item correlations matrix indicated a positive correlation between all items, exception item 10. Based on the results of the inter-item correlation, item 10 had the lowest correlation coefficient that was deleted from the questionnaire. CFA results confirmed the two-factor model of DYMUS, “dysphagia for solids” and “dysphagia for liquids”, and revealed that the item 10 was minimally contributing to the measurement of "dysphagia of solids" and internal consistency increased after the deletion of this item. Also, the solution of the 9-item Model was the best goodness-of-fit indicator for the sample. Conclusion: The Persian adjusted 9-item questionnaire was demonstrated to be a simple, consistent, valid and reliable tool for the detection of patients who need more specific instrumental evaluations of swallowing. Also, about the dysphagia is very frequent in these patients, it is important that clinicians monitor these patients carefully even in early stages of the disease‏ for early detection and planning a rehabilitation program to the prevention of serious subsequence complications.
