مقدمه: تحمل درد میگرن، پیامدهای روانشناختی متنوعی دارد که در چارچوب کاربستهای روان درمانی باید مورد توجه قرار گیرد.
هدف: هدف پژوهش حاضر تعیین اثربخشی درمان مبتنی بر پذیرش و تعهد بر انعطافپذیری شناختی، نشخوار فکری و تحمل آشفتگی در افراد دارای سردرد میگرنی بود.
روش: این پژوهش از نوع شبه آزمایشی با طرح پیشآزمون- پسآزمون با گروه کنترل بود. ۳۲ نفر مبتلا به سردرد میگرنی از دانشجویان دختر دانشگاه گیلان در نیمسال دوم تحصیلی ۹۹-۱۳۹۸، با شیوه نمونهگیری در دسترس انتخاب و به طور تصادفی در دو گروه آزمایش (۱۴n=) و کنترل (۱۸=n) جایدهی شدند. گروه آزمایش، هشت جلسه ۶۰ دقیقهای و به صورت هفتگی، درمان مبتنی بر پذیرش و تعهد گروهی دریافت کرد. قبل و پس از اتمام جلسات، هر دو گروه به پرسشنامه انعطافپذیری شناختی دنیس و وندروال (۲۰۱۰)، مقیاس پاسخهای نشخواری نولن هکسما و دیویس (۱۹۹۹) و مقیاس تحمل آشفتگی سیمونز و گاهر (۲۰۰۵) به صورت داوطلبانه پاسخ دادند. دادهها با استفاده از آزمون U مان-ویتنی تحلیل شدند.
یافتهها: نتایج آشکار ساخت گروه آزمایش در مقایسه با گروه کنترل بعد از پایان مداخله، نمرات معنادار کمتری در نشخوار فکری (۰/۰۰۵P=، ۶۲/۰۰=U) و نمرات معنادار بیشتری را در تحمل آشفتگی (۰/۰۱P، ۶۹/۵۰=U) نشان دادند. اجرای مداخله بر انعطافپذیری شناختی افراد با سردرد در گروه آزمایش تفاوت معنادار نسبت به گروه کنترل ایجاد نکرده است (۰/۷۰=P، ۱۳۳/۰۰=U).
نتیجهگیری: نتایج حاصل از پژوهش نشان داد درمان مبتنی بر پذیرش و تعهد، راهکار مناسبی برای کاهش نشخوار فکری در افراد با سردرد میگرنی و افزایش تحمل آشفتگی در افراد با دردهای مزمن است.
Introduction: Tolerating migraine pain has a variety of psychological consequences that should be considered in the context of psychotherapy applications.
Aim: The purpose of the current study was to determine the effectiveness of acceptance and commitment therapy based on cognitive flexibility, rumination and confusion tolerance in headache patients.
Method: This research was a quasi-experimental with pre-test and post-test design with a control group. Among female students of Guilan University during the second semester of the 2018-2019 academic year, the numbers of 32 patients with migraine headache were selected by accessible sampling method and then assigned randomly into two groups included experimental (n=14) and control (n=18) groups. Eight sessions in 60 minutes per week were under-treatment based on acceptance and commitment in the group. Before and after the sessions, both groups answered voluntarily using Dennis & Wenderval’s (2010) Cognitive Flexibility Inventory, Nolen-Hoeksema & Davis Ruminative Responses Scale (1999), and Simons & Gaher Distress Tolerance Scale (2005). Data were analyzed by the Mann-Whitney U test.
Results: The results revealed that the experimental group had significantly lower scores in rumination (U= 62.00, P=0.005) and higher scores in distress tolerance (U=69.50, P
Conclusion: Achieved results from the current research revealed that the acceptance and commitment therapy could be an appropriate alternative to reduce the rumination in people having migraine headache and to increase the distress tolerance in people having chronic pains.
