چکیده:
همبودی اختلال های اضطرابی و افسردگی در اکثر پژوهش های همه گیرشناسی، بین ٤٠ تا ٨٠ درصد گزارش شده است و این شرایط هزینه های بسیاری به طور مستقیم و غیرمستقیم بر جوامع تحمیل میکند. استفاده از پروتکل های درمانی شناختی رفتاری اختصاصی با توجه به چالش های برخاسته از مسئله همبودی، از ابعاد مختلف مقرون به صرفه نیست . توجه به مسئله همبودی اختلال های اضطرابی و افسردگی و پاسخ به چالش های ناشی از آن میسر نبود، مگر از طریق تدوین راهکارهایی که توجه به موضوع و اهمیت همبودی را هم در سطح نظری و هم در سطح کاربردی مدنظر داشته باشند، درمان های فراتشخیصی پیش قراول این نگاه تازه به تدوین پروتکل درمانی محسوب میشوند. رویکرد فراتشخیصی در تلاش اند تا چالش ها و معضلات درمانی ناشی از همبودی را با پرداختن به ماهیت نظری همبودی و لحاظ کردن ابعاد مشترک اختلال های هیجانی به ویژه اختلال های اضطرابی و افسردگی در تدوین پروتکل درمانی واحد برطرف کنند.
تلاش های انجام شده در این زمینه به طراحی پروتکل های فراتشخیصی متعددی منجر شد که تفاوت های آن ها در انتخاب تکنیک های تاییدشده و رایج درمان های اختصاصی شناختی رفتاری بود. اگرچه پژوهش کارآزمایی انجام شده در این زمینه از اثربخشی متوسط این پروتکل های فراتشخیصی غیرنظری و عمل گرا حمایت میکرد ولی نقطه عطفی برای ارتقا سطح اثربخشی رویکرد فراتشخیصی شکل گرفته شده بودند. پژوهشگران این زمینه سعی کردند که با منطبق کردن این پروتکل ها بر پایه های نظری و آسیب شناسی از سد این مشکل بگذرند. نقطه عطف این تلاش ها و جهت گیریها در پروتکل فراتشخیصی بارلو و همکاران (٢٠١١) نمایان است که با تکیه بر عامل «تنظیم هیجانی» و نظریه های مرتبط با آن ، اولین گام را در طراحی پروتکل های فراتشخیصی نظریه گرا برداشتند. علیرغم پیشرفت در توسعه و غنیسازی پروتکل های فراتشخیصی مبتنی بر نظریه ، مناقشات علمی اخیر در این زمینه معطوف به انتخاب فرآیند یا فرآیندهای بنیادین است .
Comorbidity of anxiety and depression disorders in most epidemiological studies has been reported between 40% and 80%, and these conditions impose many costs directly and indirectly on societies. High comorbidity of these disorders makes the use of specific protocols of cognitive behavioral therapy for these co-occurrence disorders, may not be affordable. Attention to the issue of the comorbidity of anxiety and depression disorders and the response to the challenges posed by it was not possible except through the formulation of strategies that address the issue and the importance of comorbidity both at the theoretical and applied levels, transdiagnostic treatments are prerogatives of this new perspective on the development of a therapeutic protocol. The transdignostic approach is working to resolve the comorbidity therapeutic challenges and concerns by addressing the theoretical nature of comorbidity and addressing the common dimensions of emotional disturbances, especially anxiety and depression disorders, in the development of a unified therapeutic protocol. Efforts in this field resulted in the design of several transdiagnostic protocols that differed in the selection of approved and commonly technique of cognitive-behavioral therapies. In recent years, while transdiagnostic cognitive behavioral protocols for these co-occurrence disorders have obtained favorable empirical results, but the findings of previous studies reveal moderate efficacy of such protocols. Researchers in this field tried to overcome this problem by matching these protocols on theoretical and pathological grounds. The milestone of these attempts and orientations was in the transdiagnostic protocol’s Barlow et al. (2011), as the first step in the design of theoretical transdiagnostic protocols, that based on the "emotional regulation" factor and its related theories. Despite the progress in developing and enriching the transdignostic protocols, the recent controversy is in this area focuses on the choice of process or fundamental processes.
