Patellofemoral pain syndrome is one of the common conditions that athletes suffer from, and one of the predictors of this syndrome is a decrease in the ratio of activity of the internal oblique muscle to the external oblique muscle. The aim of the present study was to compare the ratio of electromyographic activity of the internal oblique muscle to the external oblique muscle during isometric contraction of the quadriceps muscle on an isokinetic device in athletes with patellofemoral pain syndrome and healthy athletes. In this descriptive study, 16 male athletes aged 18-30 years (volleyball, handball, and taekwondo) with patellofemoral pain syndrome and 16 healthy male athletes from national teams participated voluntarily. Electromyographic activity of both muscles was recorded using surface electrodes and a telemetry electromyography system on an isokinetic device at knee flexion angles of 15, 30, and 45 degrees, and the ratio of electromyographic activity of the internal oblique muscle to the external oblique muscle was compared. An independent t-test was used to compare the ratio of muscle activity in the two groups. The results of the study showed that there was no significant difference in the electrical activity ratio of the internal oblique muscle to the external oblique muscle in either group at any of the knee flexion angles (P > 0.05). Also, the results of the study showed that the ratio of electromyographic activity of the internal oblique muscle to the external oblique muscle in both groups was higher than the numerical value of 1. However, this ratio was higher in healthy athletes than in athletes with the condition. In general, it seems that sports exercises as a preventive factor prevent disruption in the electrical activity of the internal oblique muscle, which prevents a decrease in the numerical value of the ratio of the internal oblique muscle to the external oblique muscle.

Despite researchers' belief in the lack of balance in the electrical activity of the vastus medialis obliquus and vastus lateralis muscles in patients with patellofemoral pain syndrome, the findings of the present study are consistent with the results of Davies et al.
