چکیده:
سابقه و هدف: تاکنون عوامل بسیاری در ارتباط با شروع و گسترش بیماری سارکوییدوز مطرح شده اند، اما اخیرا شواهدی مبنی بر وجود رابطه این بیماری با لشمانیا مطرح و گزارش شده است. مواد و روش ها: این مطالعه یک مطالعه مقطعی بوده که به منظور جدا سازی گونه های شایع لشمانیا از بافت پوستی بیماران مبتلا به گرانولوم سارکوییدی طراحی شده است. بدین منظور، 25 بلوک پارافینه از بیوپسی پوست بیمارانی که قبلا تشخیص گرانولوم سارکوییدی برای آنها گذاشته شده بود، جمع آوری شده و مجددا توسط یک پاتولوژیست باز خوانی و بررسی شدند. با استفاده از روشهای تشخیصی متعارف ورنگ آمیزیهای اختصاصی هیچ گونه عامل عفونی مشخصی در این نمونه ها یافت نشد. سپس به دنبال جدا سازی DNA و با استفاده از روش PCR نمونه ها از جهت وجود kDNA اختصاصی لشمانیا مورد بررسی قرار گرفتند. در این بررسی تکثیر یافتن قطعات bp620 نشانگر وجود لشمانیا ماژور و وقطعات bp830 حاکی از وجود لشمانیا تروپیکا بودند.
یافته ها: پس از انجام آزمایش در 8 مورد (32%) از نمونه ها، نتایج مطابق با باند رفرانس لشمانیا ماژور بود، درحالی که هیچکدام از نمونه ها مطابقتی با لشمانیا تروپیکا نداشتند. سن و جنس ارتباطی با وجود مثبت انگل در نمونه ها نداشت (4/0p<)
نتیجه گیری: به نظر می رسد که وجود انگل لشمانیا در مبتلایان به گرانولوم سارکوییدی شایع باشد. لذا توصیه می شود که در تمامی ضایعات بالینی مشابه با سارکوییدوز و ضایعاتی با هیستو پاتولوژی سارکوییدال گرانولوما، تست PCR برای DNA اختصاصی لشمانیا روی نمونه های بیوپسی انجام گیرد
Background and Aim: In the past، different causative agents were identified for sarcoidosis. However، recently new evidence points to an association between sarcoidosis and leishmaniasis. To further investigate this relationship، we performed a cross-sectional study on paraffin-embedded skin biopsy samples obtained from patients with clinical and histopathological diagnosis of cutaneous sarcoidosis referring to Skin Research Center of Shahid Beheshti University of Medical Sciences، in Tehran، from January 2001 to March 2005.
Materials and Methods: In order to isolate leishmania parasite from their skin lesions، we employed conventional diagnostic criteria to detect infectious agents including leishmania parasites، we re-examined twenty-five paraffin-embedded skin biopsies، diagnosed as naked sarcoidal granuloma، with no positive results obtained. Furthermore، DNA extracted from all specimens was analyzed by the commercially available polymerase chain reaction (PCR) kits
Results: Out of twenty-five، eight samples were positive for leishmania major (L. major)، whereas L. tropica was detected in none. In fact، from histo-pathological point of view، cutaneous leishmaniasis (CL) may be misinterpreted as sarcoidosis، all the more so in cases in which conventional methods fail to detect leishmania parasite.
Conclusion: In endemic areas for leishmaniasis، PCR for leishmania-specific DNA should be utilized in any granulomatous skin disease compatible with sarcoidosis، regardless of both the clinical presentation and histopathological interpretation.
