چکیده:
مداخلات والدمحور، روشی موثر برای درمان کودکان دارای اتیسم محسوب میشوند. بیشتر این مداخلات متمرکز بر نقائص ارتباطی اجتماعی هستند و کمتر به رفتارهای اضافه پرداختهاند. برای رفع این نیاز برنامه خانوادهمحور مدیریت رفتارهای اضافه اتیسم تهیه شد. هدف این مطالعه بررسی اثربخشی این برنامه روی سه تن از والدین کودکان دارای اتیسم است. این مطالعه از نوع طرحهای تکآزمودنی است که روی سه کودک دو تا سهساله دارای اختلال اتیسم انجام شده است. این والدین ۱۰ جلسه انفرادی مداخله را دریافت کردند. ارزیابیها در ۹ مرحله (سه ارزیابی خط پایه، پنج ارزیابی در مدت درمان و یک ارزیابی پیگیری) انجام شد.
ابزار مورد استفاده در پژوهش حاضر پرسشنامه تجدیدنظر شده رفتارهای تکراری، چکلیست رفتار ناهنجار و سیاهه ارزیابی درمان اتیسم بود. جهت تجزیه و تحلیل دادهها از روش تحلیل بصری و اندازه اثر استفاده شد. نتایج مطالعه نشان داد رفتارهای اضافه چون رفتار کلیشهای (۸۲/۰d=)، رفتار خودجرحی (۱۰۰/۰d=)، رفتار وسواسی (۸۵/۰d=)، رفتار آیینی (۶۹%d=)، رفتار یکنواختی (۱۰۰/۰d=)، رفتار محدود (۸۳/۰d=)، بیقراری (۷۹/۰d=)، پژواکگویی (۸۴%d=) و بیشفعالی (۲۶/۰d=) کاهش یافت. همزمان، کمبودهای رفتاری (چون گفتار، ارتباط، معاشرت) بهبود یافت، بدون آنکه هیچ مداخلهای روی کمبودهای رفتارهای اجرا شده باشد. نتایج این مطالعه از اثربخشی برنامه خانوادهمحور مدیریت رفتارهای اضافه اتیسم حمایت میکند. پیشنهاد میشود این برنامه به عنوان یکی از درمانهای زودهنگام رفتارهای اضافه، حتی مقدم بر مداخلات کمبودهای رفتاری مورد استفاده قرار گیرد تا فواید برنامههای خانوادهمحور، حاصل شود.
Parent-mediated interventions are consid-ered as an effective intervention to treat children with autism spectrum disorder (ASD). Most of these interventions focus The purpose of this study was to evaluate the effectiveness of this program on three parents of children with ASD. This study was a single-subject design and was perfo-rmed on three children with ASD aged 2 to 3 years old. Parents of these children re-ceived 10 individual sessions of FPMA-BE. 9 stages of evaluation were performed (3 evaluations in baseline, 5 evaluations during treatment and one evaluation in follow up). The instruments used in this study were the Repetitive Behavior Scale-Revise (RBS), Autism Treatment Evalu-ation Checklist and The Aberrant Beh-avior Checklist. To analyze data, visual analysis and effect size were used. The results of the study showed that BE such as stereotyped behaviors (d= 82%), self-injury (d= 100%), compulsive behaviors (d= 85%), rituals (d= 85%), sameness (d= 100%), restricted behaviors (d= 83%), irritability (d= 79%), echolalia (d= 84%) and hyperactivity (d= 26%) decreased. At the same time, SD (such as Speech/comm-unication, Sociability) improved without any intervention on behavioral deficits. Since, the results of this study support the effectiveness of the FPMABE, it is recom-mended to use this program as an early intervention for the treatment of children with ASD even prior to treatment of SD to take advantage of family-based programs.
