چکیده:
مقدمه: اختلال طیف اوتیسم، یک اختلال عصبی رشدی است که با نقص در ارتباطات اجتماعی و الگوهای رفتاری کلیشهای مشخص میشود. نشان داده شده است که دارویی دیورتیک به نام بومتاناید، تاثیر قابل توجهی را در بهبود ارتباط اجتماعی در این بیماران داشته است. نظر به اینکه یکی از شاخصهای مهم در برقراری روابط اجتماعی، تماس چشمی میباشد، هدف از انجام این مطالعه بررسی تاثیر این دارو بر تصحیح الکوی نگاه به چشم و دیگر اجزای صورت در کودکان و نوجوانان مبتلا به اختلال اوتیسم بود.
روش کار:. مطالعه حاضر یک کارآزمایی بالینی تک گروهی به صورت یک سو کور بود. شرکتکنندگان شامل 20 کودک 7 تا 15 سال مبتلا به اختلال طیف اوتیسم با عملکرد بالا بود. بعد از قطع داروهای قبلی آزمونهای مقیاس درجهبندی اوتیسم دوران کودکی (CARS)، مصاحبه تشخیصی نیمه ساختاریافته (KSADS)، معیار پاسخدهی اجتماعی (SRS)، مقیاس تجدیدنظر شده هوشی برای کودکان (WISC-R) برای هر شرکتکننده انجام شد. مداخله دارویی روزی یک میلیگرم قرص بومتاناید به صورت نیم میلیگرم صبح و عصر به مدت نود روز انجام شد. تست ردیاب چشمی هنگام نگاه شرکتکنندگان به تصاویر چهره قبل و بعد از مداخله انجام شد. متغیرهای تعداد و زمان نگاه به هر منطقه در هر تصویر ثبت گردید.
یافتهها: مقایسه قبل و بعد از مداخله تعداد نگاه به چشم برای هر بیمار برای هر تصویر و زمان نگاه به چشم برای هر بیمار برای هر تصویر تفاوت معناداری را نشان نداد (05/0P>).
نتیجهگیری: مصرف داروی بومتاناید بر روی میزان نگاه به چشم در کودکان اوتیستیک اثر معناداری نداشته گرچه علایم و شدت اوتیسم و برخی مشکلات رفتاری را بهبود داده است.
Introduction: Autism Spectrum Disorder is a neurological developmental syndrome which is characterized with a set of behavioral symptoms, including imperfection in social communication and connections, repetitive behaviors and limited activities and interests. Generally, 1 in 59 children has been identified with autism, and more often it includes boys. In Autism Spectrum Disorder, GABAergic system balance is affected and the activity of GABA is reduced. So far there has been no definite cure for decreasing autism signs. Even though a diuretic medicine, which is used for high blood pressure, dysplasia bronchopneumonia, and nephrotic syndrome, called Bumetanide has a considerable influence on curing behavioral and cognitive symptoms, understanding facial excitement and increasing brain activity in the involved area in perception, and social cognition. Since it has been shown that Bumetanide has effect on improving social communication in autistic children and also as one of the important features in making social communication is eye contact, this research is aimed to see whether this medication can reform the looking pattern at the face parts and specifically does it improve autistic children eye contact?
Method: This single group clinical trial research is conducted on 20 children participants from 7 to 15 years old with high function Autism Spectrum Disorder. After selecting and stopping the previous medicine, Childhood Autism Rating Scale (CARS), Wechsler Intelligence Scale for Children - Revised Form (WISC-R), Social Responsiveness Scale (SRS), Kiddie- Schedule for Affective and Schizophrenia (KSADS) were done for each child. A dose of 1 mg bumetanide (0.5 mg in the morning and 0.5 mg in the evening) during 90 Days was given to the participants. SRS and eye tracking tests were done as pretest and posttest. Eye tracking task was defined as two blocks of 40 images including neutral face images with defined eyes and lip and the surrounding space as the Arias of Interest (AOI). The number and Duration of fixation to each area and the total number and time of fixation in each image is registered as the variables.
Results: The comparison of pretest and posttest of each two adjusted numbers of fixation to eyes for each patient for each image, and adjusted duration of fixation to eyes for each patient for each image didn’t show any significant difference. (p
Conclusion: Using Bumetanide does not have significant effect on eye contact and on number and time of attention to eyes in autistic children, despite it has improved symptoms and severity of Autism and some behavioral problems.
