چکیده:
هدف: هدف از این پژوهش، بررسی میزان تأثیر مراقبت (درمان) های ناکافی در بیماران مبتلابه دیابت نوع 2 در ایران بر روی پیامد نهایی (عوارض میکرو و ماکرووسکولار) و هزینههای تحمیلشده در بیماران با دیابت نوع 2 است.
روش: این مطالعه یک مطالعۀ مقطعی است که در بیماران مبتلا به دیابت نوع 2 در دو مراکز دیابت (خصوصی و دولتی) در استان تهران انجام شد. با بهکارگیری مدل UKPDS و با استفاده از دادههای 5 ساله (1392-1396) بالینی جمعآوریشده از مراکز دیابت در سطح استان تهران (400 بیمار)، به پیشبینی نتایج برای مراقبت جاری پرداخته شد و سپس این مقادیر با فرض کنترل سه فاکتور اصلی هموگلوبین A1c، کلسترول با دانسیتۀ کم و فشارخون سیستولیک در سطح استاندارد برای مدت زمان 20 سال مورد پیشبینی قرار گرفت و اختلاف نتایج حاصل با نتایج اولیه بهعنوان بار اقتصادی و بار سلامت تحمیلشده بهواسطۀ مراقبت نادرست، ناکافی یا ضعیف در این بیماران، نتیجهگیری شد.
یافتهها: مقایسۀ نتایج 20 سالۀ دو سناریو نشان میدهد که نسبت به الگوی درمان جاری، امید به زندگی 4.3 سال، در بیماران با اصلاح الگوی درمان و کنترل متغیرهای اصلی، قابل دستیابی است. یافتهها نشاندهندۀ افزایش هزینههای درمانی جهت کنترل فاکتورهای ذکرشده در روش مراقبت مطلوب، به میزان 63,889,695 ریال و افزایش هزینههای کل نیز به میزان 104,852,150 است.
نتیجهگیری: نتیجۀ مدلسازی نشان داد که با توجه به جمعیت دیابتیهای نوع 2 در کشور، (حدود 5 میلیون نفر)، در حدود 30 میلیون سال و بهطور متوسط سالیانه بیش از 1.5 میلیون سال طول عمر از دست میرود. با توجه به اینکه هزینه به ازای طول عمر بهدستآمده در مراقبت کامل کمتر از 3 برابر درآمد سرانه کشور است، انجام مراقبت کامل برای بیماران دیابتی یک برنامۀ بسیار هزینه-اثربخش به شمار میرود.
Background: The purpose of this study is to evaluate the effect of inadequate (medical) care on the final outcome (micro and macrovascular effects) and imposed costs in Iranian patients suffering from type 2 diabetes. Method: The study is a cross-sectional one conducted among patients suffering from type 2 diabetes in two main diabetes clinics (a private and a public one) in Tehran province. Using the UKPDS model and 5-year clinical data collected from main diabetes centers in Tehran provinces (400 patients), outcomes for the current care were predicted, and then these values were predicted for 20 years, assuming that the three main factors of LDL, HbA1C, and systolic blood pressure at standard level have been controlled. The obtained results compared with the current care outcomes are considered economic and health burden imposed on these patients due to inappropriate, inadequate or poor care. Findings: Comparison of the 20-year results of the two scenarios shows that compared to the current treatment pattern, patients’ life expectancy can increase by 4.3 years as a result of modifying treatment patterns and controlling main variables. Findings show that treatment costs for controlling the mentioned factors increased by 63,889,695 rials and total costs increased by 104,852,150 rials in optimal care. Conclusion: The results of the model show that regarding type 2 diabetes population in the country (about 5 million people), about 30 million years and an average of over 1.5 million years annually are lost. Given that the cost of living in full care is less than 3 times the country’s per capita income, providing full care for diabetics is a highly cost-effective plan.
