چکیده:
هدف: سندرم درد کشککی رانی یکی از شایعترین مشکلات مفصل زانو میباشد. کاهش حس عمقی، درد و تغییر در الگوی فعالیت عضلات واستوس داخلی و واستوس خارجی در این گروه از بیماران دیده شده است. هر کدام از این عوامل به نحوی در توانایی حفظ تعادل موثر هستند. تعادل یکی از فاکتورهای کلیدی در انجام فعالیتهای روزمره مثل راه رفتن و دویدن است. یکی از روشهای درمانی که باعث کاهش درد و بهبود تعادل در افراد مبتلا به سندرم درد کشککی رانی میشود، چسب کینزیولوژی است. با توجه به تفاوتهای فیزیولوژیکی و آناتومیکی موجود بین زانوی زنان ومردان و اهمیت بررسی تاثیر چسب کینزیولوژی بر روی تعادل پویا در مردان مبتلا به سندرم درد کشککی رانی انجام پژوهشی در این راستا را ضروری میسازد. همچنین مطالعهای که کاربرد و تاثیر چسب کینزیولوژی را در دو گروه زنان و مردان مبتلا به سندرم درد کشککی رانی، بر روی تعادل پویا و درد مقایسه کرده باشد، یافت نشد. بنابراین هدف مطالعه حاضرمقایسه تاثیر چسب کینزیولوژی بر روی تعادل پویا و درد، بین زنان و مردان مبتلا به سندرم درد کشککی رانی یک طرفه میباشد.
روش بررسی: 30 مرد و 31 زن در گروه سنی 18 تا 40 سال با درگیری یک طرفه سندرم درد کشککی رانی و با روش نمونهگیری آسان در این مطالعه مداخلهای نیمه تجربی غیر تصادفی شرکت کردند. اندازهگیری تعادل قبل و بعد از اعمال چسب کینزیولوژی بر روی کشکک، توسط تست تعادل Y انجام شد. در مرحله اول، آزمودنی تست تعادل Y را با سه بار تکرار در هرجهت، انجام میداد؛ بیشترین مقدار عددی حاصل از سه بار اجرای حرکت مزبور در هر جهت، بر طول اندام تحتانی تقسیم شده و به عنوان نمرهی تعادل پویا و بصورت درصد در آن جهت ثبت میگردید. در مرحله دوم پس از 5 دقیقه استراحت و انجام آزمونهای اولیه، مرحله مداخله صورت میگرفت. در این مرحله، کشکک اندام درگیر، با هدف ایجاد لغزش داخلی به وسیله چسب کینزیولوژی با کشش50الی75 درصد نسبت به طول اولیه ی آن، چسب زنی میشد. ملاک موثر بودن روش چسب زنی، با تست single leg squat سنجیده میشد. به این منظور قبل و بعد از چسب زدن، فرد به مدت 10 ثانیه روی پای درگیر و با 45 درجه خمیدگی زانو، چمباتمه میزد و میزان شدت درد، توسط مقیاس دیداری درد (VAS)، با استفاده از این تست ثبت و مقایسه میگردید. در صورت کاهش تقریبی 50 درصد در میزان شدت درد، افراد وارد مرحله نهایی میشدند. در غیر این صورت، چسب زنی دوباره تکرار میشد تا میزان کاهش درد مورد نظر، به دست آید. در مرحله نهایی، افراد مورد مطالعه، تست Ybalance را مانند مرحله اول انجام میدادند.
یافتهها: مقایسه درونگروهی پارامترهای تعادل قبل و بعد از کاربرد چسب کینزیولوژی در افراد مبتلا، با استفاده از آزمون تی زوجی نشان دهنده ی افزایش مقدار عددی فاصله دستیابی بر روی خطوط رسم شده بر روی زمین در جهت قدامی، خلفی خارجی و خلفی داخلی تست تعادل Y بود که تاثیر چسب زنی کشکک در بهبود تعادل در آزمون Y balance را نشان داد (۰/۰۵P). در مقایسه بین دو گروه قبل و بعد از مداخله بر اساس آزمون تی مستقل بین زنان و مردان تفاوت معنا دار درپارامترهای تعادلی استاندارد شده، مشاهده نشد (۰/۰۵P>). همچنین در هر دو گروه زنان و مردان، با استفاده از آزمون تی زوجی، میزان شدت درد به صورت معناداری نسبت به قبل از چسب زنی کاهش یافت (۰/۰۵P)، در حالیکه در مقایسه بین دو جنس کاهش شدت درد، معنا دار بدست نیامد (۰/۰۵P>).
نتیجهگیری: یافتههای تحقیق حاضر نشان داد که استفاده از چسب کینزیولوژی میتواند باعث بهبود تعادل پویا و درد در بیماران مبتلا به سندروم کشککی رانی شود، هرچند بهنظر میرسد میزان تاثیرگذاری این روش در زنان و مردان یکسان بوده و تفاوتی ندارد.
Objective: Patellofemoral pain syndrome (PFPS) is one of the most common disorders of the knee joint. This condition is characterized by pain, poor proprioception, and altered pattern of vasti muscles activation. Each of these factors are effective in maintaining body balance, the key factor in daily activities such as walking and running. One of the treatments that reduces pain and improves balance in subjects with PFPS is kinesiology tape (KT). Considering the physiological and anatomical differences between the knee structures of both genders and the importance of studying the effect of KT on dynamic balance in men with PFPS, it seems necessary to perform a separate study on male subjects with PFPS to support the use of KT in these subjects. Moreover, there was no study to compare the effect of this intervention on dynamic balance and pain between both genders. Therefore, the purpose of the present study was to compare the effect of KT on dynamic balance and pain between males and females with unilateral PFPS.
Methods and materials: 30 male and 31 female subjects with age ranging from 18 to 40 years suffering from unilateral PFPS were recruited for this quasi-experimental and non-randomized clinical trial, using simple sampling method. The balance of each subject was assessed using Y balance test before and after KT applied on the patella. In the first stage, the subject performed the Y balance test in anterior, posteromedial and posterolateral directions, repeating three times in each direction. The maximum score obtained from three repetitions in each direction was then divided by the leg length and recorded in percentage as a dynamic balance score. In the second stage, after 5 minutes of resting time and performing initial tests, the intervention was used in these subjects. In this stage, KT was used on affected knee to provide patellar medial glide with a 50-70 percent stretch of its initial length. Effectiveness of the taping method was examined during single leg squat test. For this purpose, all subjects performed unilateral squatting on their affected leg for 10 seconds with 45 degrees of knee flexion before and after KT, while their pain level were recorded using visual analog scale (VAS). In the case of 50% reduction of pain intensity, subjects were allowed to enter the final stage; otherwise, patellar taping was repeated to obtain the appropriate pain reduction. In the last stage subjects with 50% pain reduction were selected to perform Y balance test, exactly as performed in the first stage.
Results: According to the paired t-test, within-group analysis of balance parameters before and after application of KT in the affected knees indicated an increase in the reaching distance of the anterior, posteromedial and posterolateral directions, revealing significant improvement of Y balance test, following KT(P0/05). Additionally, pain was significantly reduced in both genders after KT(P0/05).
Conclusion: These results suggest that patellar taping is an appropriate therapeutic intervention for improving dynamic balance and reducing pain level among subjects with PFPS. However, the effect of KT on dynamic balance and pain among women and men seems to be the same.
