چکیده:
افراد سالمند به دلیل مواجه شدن با شرایطی چون تغییرات جسمانی، ضعف در برابر بیماریها، معلولیت و از دست دادن خویشاوندان و دوستان (سوگ و فقدان)، زمینه تفکر در خصوص مرگ و اضطراب ناشی از آن را دارند. از این رو هدف مطالعه حاضر، بررسی اثربخشی گروه درمانی شناختی-رفتاری بر اضطراب مرگ و سلامت عمومی زنان سالمند سوگوار بود. روش این پژوهش، نیمه آزمایشی با طرح پیش آزمون-پس آزمون با گروه کنترل و دوره پیگیری بود. جامعه آماری این مطالعه، کلیه زنان سالمند دارای تجربه سوگ در مراکز سالمندان شهر کرمانشاه در سال 1397 بودند که از میان آنها، تعداد 24 نفر با در نظر گرفتن معیارهای ورود به مطالعه، انتخاب و به صورت تصادفی در دو گروه 12 نفره آزمایش و کنترل جایدهی شدند. گروه آزمایش، تحت مداخله گروه درمانی شناختی-رفتاری، در قالب 8 جلسه 90 دقیقهای، قرار گرفتند. برای جمعآوری دادههای پژوهش از پرسشنامههای سلامت عمومی گلدبرگ و هیلر، اضطراب مرگ تمپلر، تجربه سوگ بارت و اسکات و آزمون معاینه مختصر وضعیت روانی فولشتین و همکاران استفاده شد. تجزیه و تحلیل داده ها از طریق روش آماری تحلیل کوواریانس در نرم افزار SPSS نسخه 22 انجام گرفت. نتایج نشان داد بین میانگین نمرات اضطراب مرگ و مؤلفه های سلامت عمومی در دو گروه آزمایش و کنترل در پسآزمون تفاوت معنی داری وجود دارد (05/0P<). همچنین نتایج در دوره پیگیری نشان داد نمرات اضطراب مرگ و مؤلفه های سلامت عمومی در گروه آزمایش، همچنان متفاوت از نمرات گروه کنترل بود (05/0P<). بنابراین، با توجه به بالا بودن میزان اضطراب مرگ و ضعف سلامت عمومی سالمندان سوگوار، یافته های این پژوهش، ضرورت کاربرد جلسات درمانی آموزش شناختی-رفتاری در راستای کاهش اضطراب مرگ و بهبود سلامت عمومی سالمندان سوگوار را خاطر نشان می سازد.
Elderly people have a tendency to think about death and anxiety due to their physical conditions, weakness to illnesses, disability and losing relatives and friends (grief and loss). Therefore, the present study aimed to investigate the effectiveness of cognitive-behavioral group therapy on the anxiety of death and the general health of mourning elderly women. It was a semi-experimental study with a pretest-posttest design including a control group and a follow up period. Samples included all elderly women who experienced mourning attending elderly centers in Kermanshah in 2018, out of which 24 were selected based on inclusion criteria, and were randomly divided into two experimental and control groups. The experimental group received 8 sessions of 90-minute cognitive-behavioral group therapy. Goldberg and Hiller's general health questionnaire, Templer death anxiety, mourning experience of Bart and Scott, and Folstein and Partners mental examination brief test were used to collect the required data. Data were analyzed by SPSS software (version 22). The results suggested a significant difference between the mean scores of death anxiety and general health components of experimental and control groups in the post-test (P <0.05). The results from follow-up reported differences in anxiety scores and general health components of the experimental group and control group (P <0.05). Therefore, considering the high rate of death anxiety and low general health among elders, findings from the present study highlighted the necessity of using cognitive behavioral therapy sessions to reduce anxiety and improve general health of mourning elderly.
