چکیده:
مقدمه: علی رغم تلاش جامعه پزشکی میزان شکایت از کادر درمان ،رو به افزایش است. شکایت از رشته تخصصی زنان و زایمان جزء شایعترین در سالهای اخیرمی باشد برآن شدیم با بررسی میزان قصور و بررسی علل مرتبط با آن در رشته تخصصی زنان و زایمان ؛گامی موثردر پیشگیری از قصور پزشکی برداریم.
روش: مطالعه (توصیفی،تحلیلی ومقطعی) و بر اساس هدف کاربردی است وبه روش سرشماری انجام شد. پرونده های شکایت از متخصصین زنان و زایمان طی یک سال بررسی و پس از سنجش پایایی و اعتبار ؛ داده ها جمع آوری وپس ازتکمیل ، توسط نرم افزار آماری SPSS آنالیز شد .
یافته ها: از مجموع 140 پرونده بررسی شده؛ 50 مورد منجر به قصور و 90 مورد منجر به برائت شده است.از مجمـوع 50 پرونده ؛ (58%) بی احتیاطی (38%) بی مبالاتی (2%) عدم رعایت نظامات دولتی (2%) عدم مهارت؛ (4%) تأخیر در تشخیص (22%) تشخیص نادرست (18%) تأخیر در درمان (16%) عدم اندیکاسیون اقدام تشخیصی یا درمانی (8%) عدم تشخیص (8%)تکنیک نامناسب (16%) عدم رعایت موازین علمی (4%) عدم نظارت بر کار رزیدنت (2% ) قصور انتظامی (2% ) قصور پرستار ؛ رخ داده بود.
نتایج: شایعترین نوع قصور بی احتیاطی می باشد ؛افزایش دقت در اقدامات درمانی می تواند،میزان قصور را کاهش دهد . تشخیص نادرست و تاخیر در درمان و تکنیک نادرست شایعترین علت قصور می باشد برنامه ریزی لازم می تواند ،تاثیر بسزایی در کاهش خطای پزشکی داشته باشد .
Introduction: Despite the efforts of the medical community, the number of complaints from the medical staff is increasing. Complaints about obstetrics and gynecology are among the most common in recent years.
Method: The study (descriptive, analytical and cross-sectional) is applied based on the purpose and was done by census method. Complaint files from obstetricians and gynecologists within a year and after assessing the reliability and validity; Data were collected and analyzed by SPSS statistical software.
Results: Out of 140 cases reviewed; 50 cases led to negligence and 90 cases led to innocence. Out of a total of 50 cases; (58%) carelessness (38%) negligence (2%) non-compliance with government systems (2%) lack of skills; (4%) Delay in diagnosis (22%) Misdiagnosis (18%) Delay in treatment (16%) Indication of diagnostic or therapeutic action (8%) Lack of diagnosis (8%) Improper technique (16%) Failure to comply with scientific standards (4%) lack of supervision of resident work (2%) disciplinary negligence (2%) nurse negligence; Had occurred.
Results: The most common type of negligence is carelessness; increasing the accuracy of treatment can reduce the rate of negligence. Misdiagnosis and delay in treatment and incorrect technique are the most common causes of failure. Necessary planning can have a significant impact on reducing medical error.
