چکیده:
هدف: این پژوهش با هدف بررسی اثربخشی درمان شناختی-تحلیلی بر نشانگان اختلال استرس حاد و تجارب تجزیهای مصدومین حوادث جادهای انجام شد. روش: پژوهش نیمهآزمایشی با طرح پیشآزمون، پسآزمون و گروه گواه بود. تمامی مصدومین حوادث رانندگی مراجعهکننده به بخشهای ارتوپدی و ترومای بیمارستان شهدای هفتم تیر جامعه پژوهش را تشکیل دادند. تعداد 30 بیمار واجد ملاکهای ورود به پژوهش با روش در دسترس انتخاب و بهشیوه تصادفیسازی در گروههای آزمایش و گواه گمارش شدند. گردآوری دادهها علاوه بر چک لیست اطلاعات دموگرافیک با مقیاس میسیسیپی (Norris & Reed, 1997) و مقیاس تجارب تجزیهای (Bernstein & Putnam, 1986) انجام شد. گروه مداخله برنامه شناختی-تحلیلی را طی 16 جلسه بهصورت جلسات هفتگی 1 ساعته دریافت نمودند اما گروه گواه این برنامه را دریافت نکرد. دادهها با آزمونهای t (مستقل و زوجی) و تحلیل کوواریانس چندمتغیری تحلیل شدند. یافتهها: برنامه شناختی-تحلیلی بر کاهش نمره اختلال استرس حاد (02/36=F، 64/0=Eta و 05/0> P) و مولفههای آن (خاطرات رخنهکننده، مشکل در ارتباط بینفردی، ناتوانی در کنترل عاطفی و احساس افسردگی) در آزمودنیهای گروه مداخله اثربخش بوده است. بین میانگین نمره تجارب تجزیهای دو گروه نیز در دوره پسآزمون تفاوت معنیداری بود (12/23=F، 61/0=Eta و 05/0> P). نتیجهگیری: یافتههای پژوهش حاکی از کارایی این شیوه درمانی نوظهور است. بهنظر میرسد بهعلت تلفیقی بودن درمان و نقش فعال بیمار در آن برای مبتلایان به اختلالات مرتبط با آسیب و عوامل استرسزا مناسبتر است.
Purpose: The aim of this study was to evaluate the effectiveness of cognitive-analytical therapy on acute stress disorder syndrome and dissociative experiences of traffic-related injuries.Methodology: The research was quasi-experimental with pre-test, post-test and control group design. All the injured in traffic accidents referred to the orthopedic and trauma wards of Haft-e-Tir Hospital formed the research community. Thirty patients with inclusion criteria were selected by the available method and were randomly assigned to the experimental and control groups. Data were collected in addition to the Mississippi Traumatic Stress and Dissociative Experiences Scale. The intervention group received the treatment plan for 16 sessions in 1 hour weekly sessions but the control group did not receive this program. Data were analyzed by t-test (independent and paired) and multivariate analysis of covariance.Findings: Cognitive-analytical program has been effective in reducing the score of acute stress disorder and its dimensions in the subjects of the intervention group (PConclusion: Research findings indicate the effectiveness of this emerging treatment method. It seems that due to the combination of treatment and the active role of the patient in it, it is more suitable for patients with trauma and stressor-related disorder.

انـدازه تأثير درمان شناختي-تحليلي بر اختلال استرس حاد ٦٤ درصد، براي مؤلفه هاي آن در دامنه اي بين ٦١ الي ٧٣ درصد و بـراي تجارب تجزيه اي ٦١ درصد بوده است ، که مقدار آن قابل توجه ميباشد.
تايلور و همکاران (٢٠١٩) نيز همسو با اين يافته از پژوهش ، نشان دادند که درمان شناختي-تحليل بر کاهش تجارب تجزيـه اي افـراد مبـتلا بـه روان پريشـي تأثير مثبتي داشته است .
