چکیده:
زمینه: پیشرفت رواندرمانی در زمینۀ اختلال اضطراب منتشر، بستگی به شناخت عمیق علائم و عوامل مرتبط با آن دارد. با توجه به تازگی مفهوم درمان متمرکز بر هیجان، پیشینۀ مطالعات نشاندهندۀ اهمیت نقش هیجان و اثربخشی درمان در اختلالات روانشناختی است. اما آیا درمان متمرکز بر هیجان میتواند بر نگرانی و خودانتقادی افراد مبتلا به اختلال اضطراب منتشر مؤثر باشد؟
هدف: این پژوهش با هدف اثربخشی درمان متمرکز بر هیجان بر نگرانی و خودانتقادی افراد مبتلا به اختلال اضطراب منتشر انجام شد.
روش: پژوهش حاضر از نوع طرح آزمایشی تکآزمودنی با خطپایه چندگانه ناهمزمان بود. جامعه آماری پژوهش، شامل افراد مراجعهکننده به یکی از کلینیکهای روانشناسی تهران در سال 1399 بود. 4 شرکتکننده به شیوه نمونهگیری در دسترس انتخاب شده و طی 20-16 جلسه درمان متمرکز بر هیجان (تیمولاک و مکالوانی، 2018)، به پرسشنامههای نگرانی پنسیلوانیا (میر، میلر، متزگر و بورکوک، 1990) و خودانتقادی (گیلبرت، کالرک، همپل، مایلز و آیرونز، 2004) پاسخ دادند. تحلیل دادهها به روش ترسیم دیداری، درصد دادههای ناهمپوش، شاخص تغییر پایا و درصد بهبودی انجام شد.
یافته ها: درمان متمرکز بر هیجان باعث کاهش متغیر نگرانی (درصد بهبودی 61/08%) گردید و اثر آن تا پایان دوره پیگیری دو ماهه حفظ شد (درصد بهبودی 57/91%). همچنین درمان موجب کاهش متغیر خودانتقادی (درصد بهبودی 83/19%) شده که اثر آن تا پایان دوره پیگیری ماندگار بود (درصد بهبودی 79/72%). شاخص پایا در این متغیرها نیز، حاکی از معناداربودن این تغییرات بود (0/05 >P).
نتیجهگیری: با توجه به یافته های مطالعۀ حاضر در مورد اثربخشی درمان متمرکز بر هیجان بر کاهش نگرانی و خودانتقادی، این موضوع مطرح می شود که درمان با تغییر هیجان و بازسازی تجارب هیجانی نامطلوب، موجب کاهش علائم و مشکلات روانشناختی احتمالی در افراد مبتلا به اختلال اضطراب منتشر می شود.
Background: Improving the psychotherapies for generalized anxiety disorder (GAD) is dependent on a deeper understanding of the relations between symptoms and associated factors. According to the novelty of the emotion focused therapy (EFT) concept, background of literatures indicative the importance role of emotions and the efficacy of therapy in psychological disorders. But does EFT help effective reducing worry and self-criticism in individuals with GAD?
Aims: The aim of this study was performed to the efficacy of EFT on worry and self-criticism in individuals with GAD.
Methods: The present study was single case experimental design of non-concurrent multiple baseline. The statistical population of the study included individuals referred to one of the psychology clinics in tehran in 2020. 4 participant were selected by convenience sampling and during 16-20 sessions of EFT (Timulak & McElvany, 2018), were answered to worry pennsylvania questionnaire (Meyer, Miller, Metzger & Borkovec, 1990) and self-criticism (Gilbert, Clarke, Hempel, Miles & Irons, 2004). Data analysis was performed by visual inspection, percentage of non-overlapping data, reliable change index and clinically meaningful.
Results: EFT reduced the variable of worry (%61/08 improvement percentage) and its effect was maintained until the end of the two-month follow-up period (%57/91 improvement percentage). As well as, therapy caused reduce the variable of self-criticism (%83/19 improvement percentage) and the effect of which was maintained until the end of the follow-up period (%79/72 improvement percentage). Conclusion: According to results in present study about the efficacy of EFT on reduce worry and self-criticism, this issue arises that the therapy with change emotion and rebuilding unwanted emotional experiences, caused reduce symptoms and possible psychological problems in individuals with GAD.
