چکیده:
اهداف: هدف از این مطالعه، تفکیک قربانیان شیمیایی گاز خردل به دو گروه در معرض و غیر مواجهه با خردل گوگردی با استفاده از آنالیز متمایز کلاسیک و رگرسیون لجستیک دو حالته و انتخاب بهترین آنالیز بود.
ابزار و روش ها:پژوهش حاضر یک گروه تاریخی است که از سال 1384 تا 1393 انجام شده است. با روش مشاهده ای و نمونه گیری سیستماتیک، 284 نفر شامل 216 نفر از شهرستان سردشت به عنوان گروه مواجهه و 68 نفر از شهرستان رباط به عنوان گروه کنترل وارد مطالعه شدند. از همه لحاظ با گروه مورد مقایسه شدند. با استفاده از روشهای تحلیل افتراقی کلاسیک و رگرسیون لجستیک، 32 متغیر کمی مورد بررسی قرار گرفت و در نهایت این دو روش با استفاده از تحلیل منحنی سنگ مقایسه شدند. برای تجزیه و تحلیل از نرم افزار SPSS 21 استفاده شد.
یافته ها:8 متغیر معنی دار که بیشترین توانایی را در افتراق گروه ها داشتند (نسبت FEV1/FVC، تستوسترون، کلسترول، فسفر، بیلی روبین کونژوگه، تعداد گلبول های قرمز، هموگلوبین و هماتوکریت) انتخاب و وارد مدل های اصلی شدند. با استفاده از منحنی سنگ، نقاط برش متغیرها تعیین شد و مقادیر حساسیت و ویژگی برای آنالیز متمایز به ترتیب 78% و 77.5% و سطح زیر منحنی آن 81.2% بود. در افتراق گروه ها، شاخص تستوسترون قوی ترین متغیر و فاکتور بیلی روبین کونژوگه ضعیف ترین متغیر بود. در مدل رگرسیون لجستیک، متغیرهای FEV1/FVC، تستوسترون و نسبت فسفر معنیدار بود (05/0p<). حساسیت و ویژگی این مدل به ترتیب 80 درصد و 75 درصد، سطح انحنای سنگ 81 بود.
نتیجهگیری: در جداسازی قربانیان شیمیایی، روشهای آنالیز افتراقی کلاسیک و رگرسیون لجستیک نتایج مشابهی دارند، اما تحلیل افتراقی به دلیل ارائه عملکرد تشخیصی مدل مناسبتری است.
The aim of this study was to separate the chemical victims of mustard gas into exposed to and non-exposed groups to sulfur mustard using classical discriminant analysis and two-state logistic regression and selection of the best analysis.
Instruments & Methods The present study is a historical group that was conducted from 2005 to 2014. By observation method and systematic sampling, 284 people were included in the study including 216 people from Sardasht City as an exposed group and 68 people from Rabat City as a control group who were in all respects compared to the case group. Using classical discriminant analysis and logistic regression methods, 32 quantitative variables were examined and finally these two methods were compared using rock curve analysis. SPSS 21 software was used for analysis.
Findings The 8 significant variables that had the highest ability to differentiate the groups (FEV1/FVC ratio, testosterone, cholesterol, phosphorus, conjugated bilirubin, red blood cell count, hemoglobin and hematocrit) were selected and entered into the main models. Using the rock curve, the cutting points of the variables were determined and the sensitivity and specificity values for discriminant analysis were 78% and 77.5%, respectively, and its sub-curved surface was 81.2%. In differentiating the groups, testosterone index was the strongest variable and conjugated bilirubin factor was the weakest variable. In logistic regression model, FEV1/FVC, testosterone and phosphorus ratio variables were significant (p<0.05). The sensitivity and specificity of this model were 80% and 75%, respectively, the rock curvature level was 81.4% and the value of R^2 was 0.308.
