چکیده:
هدف: التهاب فاسیای کف پا متداولترین علل بروز درد پاشنه می باشد. استفاده از اسپیلنتهای شبانه مانع کنتراکچر عضلات و فاسیای کف پا شده و در درمان این عارضه کاربرد دارد. هدف این مطالعه بررسی اثرات استفاده از اسپیلنت شبانه کشش عضلات کالف با اسپیلنت شبانه کشش فاسیای کف پایی بر دامنه حرکات مفاصل مجموعه مچ و پا، شدت درد و کیفیت زندگی در افراد مبتلا به التهاب کف پایی بود. روش بررسی: در یک مطالعه بالینی با گروههای موازی، 30 نفر (27 خانم) فرد مبتلا به عارضه در سه گروه اسپیلنت کشش کالف، اسپیلنت کشش فاسیای کف پا و گروه کنترل (پد پاشنه) تقسیم شدند. در جلسه پیش آزمون (قبل از مداخلات)، متغیرهای مطالعه شامل دامنه حرکتی مچ پا، مفصل متاتارس شست پا، شدت درد و کیفیت زندگی به ترتیب با ابزار گونیومتری، مقیاس بصری درد و نسخه فارسی پرسشنامه SF-36 اندازه گیری شد. پس از 4 هفته، جلسه پس آزمون برگزار و متغیرهای مطالعه دوباره اندازه گیری شدند. برای تحلیل داده ها از آزمون آنالیز کوواریانس استفاده شد. یافته ها: آزمون تحلیل کوواریانس نشان داد اختلاف معنادار آماری بین گروههای مطالعه در پس آزمون شدت درد ( η2 =0/44, p=0/001, f=9/35) و میزان اکستنشن مفصل متاتارسوفالنژیال اول (η=0/27, p=0/017, f= 4/762) وجود داشت. در متغیرهای نمره کیفیت زندگی و میزان دورس فلکسیون مچ پا اختلاف معنادار آماری در سه گروه مطالعه مشاهده نگردید (05/0 p). پساآزمون بنفرونی نشان داد هر سه گروه مطالعه در پس آزمون شدت درد کمتری در مقایسه با پیش آزمون ابراز کردند (0/05> p). استفاده از اسپیلنت کشش کالف موجب افزایش معنادار دامنه اکستنشن متاتارس شست نیز گردیده بود (0/02= p). نتیجه گیری: هر سه نوع ارتوز شامل پد پاشنه و دو نوع اسپیلنت شبانه در بهبود درد ضایعه موثر بوده اند. همچنین نتایج نشان داد که استفاده اسپیلنت کشش کالف تاثیر بیشتری در مقایسه با اسپیلنت کشش فاسیا بر بهبودی شدت درد و انعطاف پذیری بافتها داشته است.
Objective: Plantar fasciitis is the most common cause of heel pain. The night splints are used to prevent nocturnal contractures happened in the calf muscles and plantar fascia. The objective of this study was to investigate the effects of using tension night splint for calf and plantar fascia on the range of motion of foot and ankle complex, pain severity, and quality of life.
Methods: 30 people (27 female) with plantar fasciitis volunteered in a parallel-groups clinical study. Participants were allocated to three study groups, including tension calf splint, tension plantar fascia splint, and control group (heel pad). The study outcomes were the range of motion of foot and ankle complex, pain severity, and quality of life measured with a goniometer, visual analog scale, and SF-36 questionnaire, respectively. The post-session was four weeks after participants received their interventions. The analysis of covariance statistical test was used to analyze the within and between-group differences.
Results: The analysis of covariance test showed statistically significant differences for pain (f=9.35, p=0.001, η2=0.44) and extension of the first metatarsophalangeal (f=4.76, p=0.017, η2=0.27). There was no statistically significant difference for the quality of life and the ankle dorsiflexion range between groups (p>0.05). Conclusion: all three orthoses used in this study significantly reduced the pain in people with plantar fasciitis. The results also suggest that tension calf splint has greater effectiveness in improving pain severity and soft tissue flexibility compared to tension fascia splint.
