چکیده:
اهداف لکنت اختلالی گفتاری است که بهصورت وقفههای مکرر و غیرطبیعی در جریان گفتار اتفاق میافتد و معمولاً بهصورت تکرار، کشیدهگویی و گیر در جریان هوا یا صداسازی رخ میدهد. علیرغم تئوریهای متعدد تاکنون علت لکنت بهطور کامل مشخص نشده و عوامل مختلفی برای سببشناسی آن مطرح شده است که از آن جمله میتوان به جنبههای شناختی و زبانی اشاره کرد. برطبق فرضیه چرخه معیوب، افزایش ناروانی گفتار در نتیجه بیشازحد توجه کردن روی گفتار است. حال در این مطالعه برآنیم تا تأثیر کاهش توجه بر گفتار را با استفاده از تکالیف دوگانه در 2 حوزه کلامی و غیرکلامی بر کودکان دارای لکنت بسنجیم. هدف از این مطالعه بررسی تأثیر تکالیف دوگانه بر شدت لکنت در کودکان مدرسهرو بود. روش بررسی در این مطالعه مقطعیمشاهدهای آزمودنیها شامل 39 کودک 8 تا 12 سال (با میانگین 10/25 سال) مبتلا به لکنت رشدی که 5 دختر (12/8 درصد) و 34 (87/2 درصد) پسر بودند بهصورت نمونهگیری دردسترس از مدارس ابتدایی مناطق 4 و 8 و کلینیکهای گفتاردرمانی شهر تهران انتخاب شدند. طراحی تکلیف دوگانه با استفاده از برنامه ویژوال بیسیک انجام شد. در طی پژوهش انجامشده 4 نمونه گفتاری از افراد گرفته شد. نمونه دوم و سوم بهصورت تکلیف دوگانه (اضافه شدن تکلیف کلامی یا غیرکلامی) و تکلیف اول و چهارم بهصورت تکلیف منفرد طراحی شد. شدت لکنت آزمودنیها در هریک از نمونههای گفتاری براساس درصد هجاهای لکنتشده (SS%) به دست آمد و دادهها وارد نسخه 20 نرمافزار SPSS شد. با توجه به توزیع غیرطبیعی دادهها، از مدل خطی تعمیمیافته با روش معادلات براورد تعمیمیافته و آزمون بونفرونی جهت تعیین تفاوت شدت لکنت در نمونههای گفتاری استفاده شد. یافتهها نتایج آزمون معادلات برآورد تعمیمیافته نشان داد شدت لکنت در 4 نمونه گفتاری تفاوت دارد ( P=0/015). کمترین و بیشترین میانگین شدت لکنت در 39 آزمودنی بهترتیب مربوط به تکلیف دوگانه غیرکلامی (14/85) و تکلیف منفرد ابتدایی (17/11) بود. در مقایسه دوبهدوی شدت لکنت مشخص شد میانگین شدت لکنت در تکلیف منفرد انتهایی بهطور معنیداری کمتر از تکلیف منفرد ابتدایی است (05/P0). نتیجهگیری یافتههای این تحقیق نشان داد انجام تکالیف دوگانه توسط کودکان دارای لکنت، موجب کاهش شدت لکنت آنان میشود. به عبارت دیگر معطوف کردن توجه کودکان موردمطالعه در این پژوهش به تکالیف کلامی و غیرکلامی در حین صحبت آنها به کاهش ناروانی آنان منجر شد.
Objective: Stuttering is a speech disorder that occurs with frequent and abnormal disruptions in speech, such as sound repetition, sound prolongation, and sound or airflow blockage. Although various hypotheses and factors have been introduced including cognitive and linguistic factors, the etiology of stuttering has not been fully understood. According to the vicious circle hypothesis, increased speech disfluency results from hypervigilance to speech. However, this study aimed to measure the effect of reduced attention on speech through verbal and non-verbal dual tasks among school children with stuttering. Furthermore, the effect of dual tasks on the severity of stuttering is investigated among them.
Method: In this cross-sectional study, the participants included 39 children (5 girls 12.8%) and (34 (87.2%) boys) aged 8-12 years (10.25±1.35) with developmental stuttering selected via convenience sampling from primary schools in districts 4 and 8 and speech therapy clinics in Tehran, Iran. Dual tasks were designed using the Visual Basic program. During the research, four speech samples were recorded for each participant. The second and third samples were designed as a dual task (addition of verbal or non-verbal tasks) while the first and fourth samples were designed as a single task. The participant’s severity of stuttering in each sample was calculated based on the number of stuttered syllables (SS%) and data were entered into SPSS-20 software for analysis. Given the non-normal distribution of data, the generalized linear model with the method of generalized estimating equation (GEE) and Bonferroni test were used to determine the difference in stuttering severity among speech samples.
Findings: The GEE results showed that the severity of stuttering was different among the four speech samples (P-value = 0.015). The lowest and highest mean stuttering severity among the 39 participants pertained to non-verbal dual task (14.85) and the initial single task (17.11), respectively. A pairwise comparison of stuttering severity showed that the mean stuttering severity in the final single task was significantly lower than the initial single task (P-value0.05).
Conclusion: Findings of this study showed that severity of stuttering reduces when children with stuttering perform dual tasks. In other words, focusing children’s attention on verbal and non-verbal tasks while talking led to a reduction in their disfluency.
