چکیده:
مقدمه: با وجود تاکید در اسناد بالادستی و سیاستهای کلی تامین اجتماعی، هنوز هم فرآیند پیادهسازی پرونده الکترونیک سلامت (EHR) در نظام سلامت ایران به صورت کامل اجرا نشده است. هدف از این مطالعه، شناسایی و تبیین موانع و راهکارهای عملیاتی برای پیادهسازی EHR در نظام سلامت ایران است.روش: این پژوهش از نوع مروری حیطهای (Scoping Review) بود که با استفاده از چارچوب Arkesy و O’Malley در 6 گام شامل شناسایی سؤال تحقیق، شناسایی مطالعات مرتبط، انتخاب/غربالگری مطالعات، حیطهبندی/تقسیمبندی دادهها، جمعبندی، خلاصهسازی و گزارش نتایج و ارائه راهنمایی و توصیههای عملی انجام شد. پایگاههای اطلاعاتی داخلی SID، Magiran، Irandoc، و پایگاههای انگلیسی زبان PubMed, Scopus, Web of Science, ProQuest, Google scholar, جستجو شدند. مقالات انتخاب شده، با روش تحلیل محتوا با استفاده از نسخه 10 نرمافزار MAXQDA تحلیل شدند.یافتهها: پس از انجام غربالگریها، 32 مقاله معیارهای ورود به این مطالعه را داشتند. موانع و راهکارهای عملیاتی در سه سطح ماکرو (سیاستگذاری و مدیریت سلامت)، مزو (مراکز ارائه دهنده خدمات سلامت) و میکـرو (ارائهدهندگان خدمات سلامت) طبقهبندی شدند. مدیریت و رهبری مطلوب، تهیه استانداردهای لازم، فرهنگسازی در جامعه، آماده کردن فرآیندها، آموزش کارکنان قبل از اجرای طرح، مشارکت کارکنان در اجرای طرح و حمایت مالی دولت از مهمترین راهکارهای عملیاتی برای اجرای EHR در ایران بود.نتیجهگیری: پیادهسازی EHR در نظام سلامت ایران با موانعی روبرو است که باید اقداماتی به طور هماهنگ و همزمان در سطوح سیاستگذاری و مدیریت سلامت، مراکز ارائهدهنده و ارائهکنندگان خدمات سلامت انجام شود تا کارایی و اثربخشی ارتقاء پیدا کند.
Introduction: Despite the emphasis on overhead documents and general social security policies, the process of implementing the Electronic Health Records (EHR) in the Iranian health system has not yet been fully implemented. The purpose of this study is to identify and explain operational barriers and solutions for the implementation of EHR in the Iranian health system. Method: This study was a scoping review, using the Arkesy and O'Malley framework in 6 steps including identifying research questions, identifying related studies, selecting/screening studies, data area/division of data, summarizing, reporting results, practical guidance and recommendations. Iranian databases including SID, Magiran, irandoc, and English -language databases including PubMed, Scopus, Web of Science, ProQuest and Google scholar were searched. Selected articles were analyzed using Maxqda software (version 10). Findings: After screening, 32 articles were selected. Operational barriers and solutions were classified into three levels of macro (policy and health management), Messo (centers of health services) and micro (health service providers). Desirable management and leadership, providing the necessary standards, community-building, preparing processes, training before implementation of the project, employee participation in implementation of the plan, and government financial support were among the most important operational strategies for EHR implementation in Iran. Conclusion: The implementation of EHR in the Iranian health system faces obstacles that need to be taken in coordinated and simultaneously at the level of health policy and management levels, providing centers and health service providers to improve efficiency and effectiveness.
