چکیده:
مقدمه: قاعدگی رخدادی حائز اهمیت و آغازگر تولید مثل در زندگی زنان میباشد، اما برخی زنان از اختلال سیکل قاعدگی در رنج هستند. پژوهش حاضر با هدف تعیین تاثیر رفتاردرمانی شناختی بر علایم سندرم پیش از قاعدگی و اختلال ملال پیش از قاعدگی انجام شد. روش کار: پژوهش حاضر نیمهتجربی با طرح پیشآزمون_پسآزمون و پیگیری با گروه گواه بود. جامعه پژوهش کلیه زنان مراجعهکننده به کلینیکهای زنان و زایمان شهر همدان در سال 1401 بودند، که از میان آنان 40 نفر با روش نمونهگیری هدفمند انتخاب و به صورت تصادفی در 4 گروه 10 نفره (دو گروه آزمایش و دو گروه گواه) جایگزین شدند. آزمودنیها با پرسشنامه غربالگری علایم قبل از قاعدگی (PSST) مورد ارزیابی قرار گرفتند. گروههای آزمایش طی 8 جلسه 60 دقیقهای به صورت هفتگی در مداخلات رفتاردرمانی شناختی شرکت کردند اما گروههای گواه هیچ مداخلهای دریافت نکردند. یک ماه و نیم بعد مرحله پیگیری انجام گرفت. جهت تحلیل دادهها از نرمافزار SPSS-24 و آزمون تحلیل کوواریانس تکمتغییری استفاده شد. یافتهها: نتایج نشان داد رفتاردرمانی شناختی در کاهش علایم سندرم پیش از قاعدگی در مرحله پسآزمون (8/33=F، 0/01P <) و در مرحله پیگیری (28/05=F، 0/000P <) موثر بود. همچنین این درمان در کاهش علایم اختلال ملال پیش از قاعدگی در مرحله پسآزمون (9/36= F، 0/007P <) و در مرحله پیگیری (21/97=F، 0/000P <) موثر واقع شد. نتیجهگیری: با توجه به نتایج، رفتاردرمانی شناختی در کاهش علایم اختلالات قاعدگی موثر بود و تاثیرات آن به مدت یک ماه و نیم پس از مداخله ماندگار بود. لذا پیشنهاد میشود که از رفتاردرمانی شناختی به عنوان یک ابزار موثر در راستای ارتقا سلامت جامعه، در تدوین برنامههای درمانی، ارائه برنامههای آموزشی بهداشت زنان، و بهبود سلامت روان زنان با اختلالات قاعدگی استفاده گردد.
Introduction: Menstruation is an essential event and the beginning of reproduction in a woman's
life. However, some women suffer from menstrual disorders. This study aimed to determine the
effect of cognitive behavioral therapy on the symptoms of premenstrual syndrome and premenstrual
dysphoric disorders.
Methods: This quasi-experimental study with pre-test, post-test and follow-up phases was conducted
with a control group. The statistical population was all women who were referred to the
gynecological and maternity clinics of Hamedan, Iran, in 2022. Forty people were selected by
purposeful sampling method and randomly divided into four groups of ten people (two experimental
and two control groups). The experimental groups participated in cognitive behavioral therapy
during eight weekly sessions of 60 minutes, but the control groups did not receive any intervention.
A month and a half later, the follow-up phase was done. Data analysis was done by using SPSS-24
statistical software using univariate covariance analysis.
Results: The results showed that cognitive behavioral therapy was effective in reducing premenstrual
syndrome symptoms in the post-test phase (F=8.33, P<0.01) and the follow-up phase
(F=28.05, P<0.000). Furthermore, this treatment was effective in reducing the symptoms of
premenstrual dysphoric disorder in the post-test phase (F=9.36, P<0.007) and follow-up phase
(F=21.97, P<0.000).
Conclusion: Cognitive behavioral therapy was effective in reducing the symptoms of menstrual
disorders, and its effects lasted for one and a half months after the intervention. Therefore, cognitive
behavioral therapy should be used as a tool to improve society's overall health. Recommendedly,
treatment programs should be established, comprehensive women's health education should be
offered, and initiatives to enhance the mental health of women experiencing menstrual disorders
must be prioritized.
