چکیده:
مقدمه: با توجه به اهمیت بررسی علائم جسمانی در بیمارن مبتلا به صرع، پژوهش حاضر با هدف بررسی مدل ساختاری علائم جسمانی بر اساس حمایت اجتماعی ادراکشده و ترومای کودکی با میانجیگری افسردگی، اضطراب و استرس در بیماران مبتلا به صرع انجام شد. روش پژوهش: این مطالعه از نظر هدف کاربردی و از نظر شیوه اجرا توصیفی از نوع همبستگی بود. جامعه پژوهش بیماران مبتلا به صرع کشور ایران در سال 1400 بودند که 260 نفر از آنها با روش نمونهگیری تصادفی ساده بهعنوان نمونه انتخاب شدند. ابزارهای پژوهش شامل پرسشنامه علائم جسمانی (کروئنکی و همکاران، 2002)، مقیاس حمایت اجتماعی ادراکشده (زیمت و همکاران، 1988)، پرسشنامه ترومای کودکی (برنشتاین و همکاران، 2003) و مقیاس افسردگی، اضطراب و استرس (لاویباند و لاویباند، 1995) بود. دادهها با روش مدلسازی معادلات ساختاری در نرمافزارهای SPSS-24 و AMOS-26 تحلیل شدند. یافتهها: یافتهها نشان داد که مدل علائم جسمانی بر اساس حمایت اجتماعی ادراکشده و ترومای کودکی با میانجیگری افسردگی، اضطراب و استرس در بیماران مبتلا به صرع برازش مناسبی داشت. همچنین، حمایت اجتماعی بر افسردگی، اضطراب و استرس و علائم جسمانی اثر معنادار منفی، ترومای کودکی بر افسردگی، اضطراب و استرس اثر معنادار مثبت و افسردگی، اضطراب و استرس بر علائم جسمانی اثر معنادار مثبت داشت (0/05>P)، اما ترومای کودکی بر علائم جسمانی اثر معناداری نداشت (0/05P). نتیجهگیری: طبق نتایج این مطالعه برنامهریزی برای کاهش علائم جسمانی در بیماران مبتلا به صرع از طریق افزایش حمایت اجتماعی و کاهش افسردگی، اضطراب و استرس ضروری میباشد.
Introduction: Considering the importance of examining somatic symptoms in patients with epilepsy, the present study was conducted with the aim of investigating the structural model of somatic symptoms based on perceived social support and childhood trauma with the mediating of depression, anxiety and stress in patients with epilepsy. Methods: This study in terms of purpose was applied and in terms of implementation method was descriptive from type of correlation. The research population was the patients with epilepsy of Iran in the 2021 year, which number of 260 people of them were selected as a sample by simple random sampling method. The research tools were included somatic symptoms questionnaire (Kroenke et al., 2002), perceived social support scale (Zimet et al., 1988), childhood trauma questionnaire (Bernstein et al., 2003) and depression, anxiety and stress scale (Lovibond and Lovibond, 1995). Data were analyzed by structural equation modeling in SPSS-24 and AMOS-26 software. Results: The findings showed that the model of somatic symptoms based on perceived social support and childhood trauma with the mediating of depression, anxiety and stress in patients with epilepsy had a good fit. Also, social support had a significant negative effect on depression, anxiety and stress and somatic symptoms, childhood trauma had a significant positive effect on depression, anxiety and stress, and depression, anxiety and stress had a significant positive effect on somatic symptoms (P0.05). In addition, social support with the mediating of depression, anxiety and stress had a significant negative effect on somatic symptoms and childhood trauma with the mediation of depression, anxiety and stress had a significant positive effect on somatic symptoms (PConclusion: According to the results of this study, it is necessary to plan to reduce somatic symptoms in patients with epilepsy by increasing social support and reducing depression, anxiety and stress.
