چکیده:
زمینه و هدف: هدف از این مقاله بررسی تظاهرات بالینی و علل سردرد مزمن بدنبال ضربه به سر می باشد. علت، نحوه درمان و جنبه های قانونی این نوع سردرد هنوز مشخص نیست. برخی مطالعات بر جنبه های روانپزشکی در ایجاد این سردرد اشاره کرده، ولی مطالعات دیگر اساس جسمانی را پذیرفته اند. این مطالعه با توجه به پیچیدگی های بالینی و مشکلات قانونی حاصله و با عنایت به شیوع زیاد ضربات سر در ایران و همچنین محدود بودن مطالعات بالینی در این زمینه انجام شد.
روش بررسی: این مطالعه طی مدت حدود 5/2 سال در درمانگاه های مغز و اعصاب و روانپزشکی بیمارستان های قائم و ابن سینا مشهد انجام شد. بیماران شامل افراد با شکایت ضربه به سر و سردرد بودند و براساس معیارهای انجمن بین المللی سردرد - ویرایش دوم - تشخیص سردرد مزمن بدنبال ضربه مطرح شده بود.
یافته ها: در بررسی انجام شده 79 بیمار معیارهای سردرد مزمن بدنبال ضربه سر را داشتند. 56% بیماران زن بودند و سن متوسط بیماران در زمان مراجعه 32 سال بود. سردرد رابطه معکوس با شدت ضربه داشت. 62 بیمار جهت پیگیری طولانی مراجعه کردند. 55% این افراد پاسخ درمانی خوبی نداشتند و در نهایت حاضر به پیگیری تکمیلی نشدند. 10 بیمار از طرف مراجع ذیصلاح قانونی معرفی شده بودند که در 4 نفر معیارهای سردرد مزمن به دنبال ضربه وجود داشت.
نتیجه گیری: مطالعه حاضر نتایج مشابه با سایر مطالعات انجام گرفته در این زمینه را نشان داد. سردرد به دنبال ضربه به سر در اکثر موارد با ضربه شدید همراه نبود؛ و به نظر می رسد آسیب درحد میکروسکوپی و روابط بین سلولی ایجاد شده بود. تعیین جایگاه های علمی و از طرف دیگر ضوابط قانونی دراین نوع سردرد مهم و انجام کارآزمایشی بالینی توصیه می گردد.
Background & Aims: The aim of this study was to assess the clinical manifestation and causes of chronic post-traumatic headache. Several publications have emphasized the relationship between head trauma and chronic post-traumatic headache. However، there is little known about the etiology (psychiatric or organic)، treatment and particularly legal issues of chronic post-traumatic headache. In spite of the high number of accidents and head trauma this study was preformed in Iran.
Materials & Methods: This was a retrospective study for 2.5 years in Neurology and Psychiatry outpatient clinic of Ghaem and Avicenna Hospitals. Patients included according to the diagnostic criteria of the primary headaches in The International Headache Classification 2nd Edition. Findings: The clinical presentation of chronic post-traumatic headache in 79 patients was analyzed. 56% of patients were women. The average age of our patients was 32. Headache had an inverse relation with the severity of trauma. 62 patients agreed to follow in this study. 55% had a bad response to treatment and finally did not continue it. 10 patients were referred form the forensic department and 4 fulfilled the criteria of chronic post-traumatic headache.
Conclusion: The results of this study were similar to previous researches. Most of the time headache was not associated with a severe head trauma. It is possible that microscopic brain injury in the level of cellular relation is responsible for chronic post-traumatic headache. Further studies and clinical trials are necessary to clarify the scientific bases and also legal issues of chronic post-traumatic headache.
