چکیده:
مقدمه : نظام سلامت ایران در محیط بسرعت در حال تغییر؛ و با چالش های متعددی در زمینه عدالت، مناسبت و اثربخشی مواجه است. در پاسخ به این فشارهای فزاینده، برای کاهش هزینه ها و افزایش کارآیی، کوچک سازی واحدهای ستادی بهداشت در تبریز طراحی و اجرا گردید.این پژوهش با هدف یافتن شواهد و پاسخهایی مستند به چالشهای موجود در کلان شهرهای کشور در خصوص تعدد و وحدت مدیریتی بهداشت مراکز استانهای مربوطه طراحی و اجرا شده است. و طی آن مدیریتهای ستادی جداگانه بهداشت تبدیل به مرکز واحد شدند.
روش بررسی :این مطالعه از نوع کاربردی است. جامعه و نمونه پژوهش ستادهای بهداشت دوگانه قبل و مرکز واحد بعد از کوچک سازی به همراه واحدهای تابعه ( از نظر تعداد پرسنل ستادی، هزینه ها، کمیت و کیفیت خدمات ارائه شده و رضایت شغلی کارکنان) بود.
نتیجه گیری: گرچه تعداد پرسنل ستادی با کوچک سازی از 248 نفر به 148 نفر کاهش یافت و هزینه های جاری نیز از 47/67 درصد سال 82 به 93/69 درصد در سال 83 رسید. با اینحال کمیت و کیفیت خدمات بهداشتی ارائه شده ( مهمترین وظیفه مرکز) نه تنها دچار نقصان نشد، بلکه در مواردی بهبود هم یافت. و رضایت از کارپرسنل شاغل قبل و بعد از کوچک سازی تغییر معنی داری پیدا نکرد. بنابراین کوچک سازی را می توان به عنوان استراتژی اصلاحات مدیریتی در سایر کلان شهرهای کشور و بخش های دانشگاه نیز اجرا نمود.
Introduction: Health system of Iran has been challenged in the so many ways such as equity، relevance، and efficacy in a changing environment rapidly. To answer to this increasing challenge، and to decrease the costs، and increase the outputs، minifying units of health system network in Tabriz Metropolitan have been designed as an experience. Present research have been designed and implemented to find the answers and evidences documented witnesses of metropolitan cities in case of unifying or multiple administrations for provincial health centers and meanwhile separated administration of health centers headquarter unified.
Methods: This is an applied study. The community، and the sample of research are health centers headquarters before and after the minifying units.( based on personnel، costs، performances qualities and quantities، and employees' job satisfactions).
Results: Even though the number of personnel have been reduced from 248 to 148 after minifying، and the cost flow have been changed from 67/47 % (2002) to 69/93% (2003) qualities and quantities of health performances( the most important tasks of health centers) not only do not decreased، but in some cases have been improved. The personnel s' job satisfactions have not changed either before of after the minifying.
Conclusions: Therefore the minifying procedures may be used as an improvement strategy in metropolitan cities and universities units.
