چکیده:
سابقه و هدف: ایجاد مقاومت در میکروارگانیزم ها نسبت به آنتی بیوتیک ها، یکی از مشکلات جدی در زمینه طب عفونی می باشد که علت عمده آن تجویز نامناسب آنتی بیوتیک توسط پزشکان است. از آنجایی که درباره الگوی تجویز تجربی (Empiric) آنتی بیوتیک در کشور اطلاعات چندانی وجود نداشت، این مطالعه با هدف بررسی الگوی تجویز تجربی آنتی بیوتیک در یکی از بیمارستان های تابعه دانشگاه شهید بهشتی، انجام شد.
مواد و روش ها: این مطالعه توصیفی- مقطعی، در سال 1385 در اورژانس بیمارستان مورد مطالعه، با بررسی پرونده 200 نفر از بیمارانی که حداقل یک آنتی بیوتیک در اورژانس دریافت کرده بودند، انجام شد. پزشکان مجری طرح پس از گرفتن شرح حال و معاینه بیماران فوق، فرم اطلاعاتی مطالعه را با توجه به مستندات پرونده بیماران در اورژانس، تکمیل کردند. در مورد ارزیابی مناسب بودن آنتی بیوتیک تجربی تجویزشده، مرجع، آخرین چاپ کتاب بیماری های عفونی مندل (سال 2005) و پروتکل مصوب کمیته کنترل عفونت بیمارستان محل مطالعه بود.
یافته ها: از بین 200 پرونده بررسی شده که در آنها حداقل یک آنتی بیوتیک برای بیمار تجویز شده بود، در 108 بیمار (54%)، آنتی بیوتیک تجویزشده اندیکاسیون نداشت و از 92 موردی که تجویزآنتی بیوتیک اندیکاسیون داشت، در 55 مورد (8/59%)، آنتی بیوتیک به صورت صحیح (از نظر مقدار، نوع و فاصله) تجویز شده بود. نتیجه گیری: میزان تجویز آنتی بیوتیک بدون وجود اندیکاسیون و انتخاب نابجای آنتی بیوتیک در اورژانس بیمارستان محل مطالعه زیاد بود. بنابراین، اقداماتی از قبیل آموزش دوره ای پزشکان و نظارت بر تجویز آنتی بیوتیک ها لازم است.
Background and Aim: Resistance in microorganisms is a serious problem in infectious medicine which the most reason is inappropriate antibiotic prescription by physicians. As there was not any information about the pattern of empiric antibiotic prescription in Iran، this study designed for recognition of antibiotic prescription pattern in one of emergency departments in a hospital affiliated to Shahid Beheshti medical university. Materials and Methods: This cross sectional study was performed during the years 2006-7 in the emergency department of the above mentioned hospital on 200 patients whom at least received one antibiotic. Physicians، after taking history and physical examination، completed the information form according to the patients chart in emergency department. Our references on appropriateness of empiric prescribed antibiotics were a) textbook of "Mandell's Principles and Practice of Infectious diseases، 6th Edition; 2005"، and b) protocols from the "Infection Control Committee" of the same hospital. Results: Among 200 patients to whom at least one antibiotic was prescribed، for 108 cases (54%)، prescribed antibiotic was without indication and among 92 cases which antibiotic was indicated، in 55 cases (59.8%) the prescribed antibiotic was appropriate، i.e.، the prescribed antibiotic was appropriate by type، dose and interval. Discussion: according to the results، it is prudent that the prescription of antibiotics without clear indication and wrong selection of empiric antibiotics in this hospital's emergency department were disproportionately high. Thus it seems that using solutions like periodic education programs for physicians and supervision for prescribed antibiotics are mandatory.
