چکیده:
سابقه و هدف: تغییرات عملکرد تیرویید مادر در زمان بارداری پیامدهای مهمی بر جنین/نوزاد خواهد داشت. اختلالهای بارز در عملکرد تیرویید در 2 تا 3 در صد بارداریها دیده میشوند اما موارد تحت بالینی آن شایع تر هستند و اغلب بدون تشخیص باقی می مانند. شیوع پرکاری تیرویید در دوران بارداری حدود 3/0 درصد بوده که در صورت عدم تشخیص و درمان مناسب به عوارض جدی و غیر قابل جبران برای مادر و جنین منجر می شود. هدف این مطالعه مروری، بررسی جنبه های تشخیصی و درمانی پرکاری تیرویید در دوران بارداری و شیردهی است.
مواد و روشها: از واژگان «پرکاری تیرویید و بارداری»، «داروهای ضد تیرویید و بارداری»، «ید رادیواکتیو و بارداری»، «پرکاری تیرویید و شیردهی» و«داروهای ضد تیروییدی و شیردهی» به تنهایی و همچنین با اضافه کردن «جنین» و «مادر» و «نوزاد» برای ردیابی و جستجو در MEDLINE بین سالهای 1960 تا 2010 میلادی استفاده شد. تمام مطالعات از نوع کار آزمایی بالینی و مورد-شاهدی که دارای طراحی مناسب بودند و مقالات مروری که کیفیت نگارشی مطلوبی داشته و نویسنده حداقل به چهار مقاله مربوط به خود در فهرست مراجع مقاله اشاره کرده بود، استفاده شد. در جستجوی منابع از کتابدار متخصص در این زمینه کمک گرفته شد.
یافته ها: داروهای ضد تیروییدی درمان انتخابی پرکاری تیرویید در زمان بارداری و شیردهی می باشند. از تمام انواع داروهای خوراکی ضد تیروییدی در این دوران می توان استفاده کرد. از آنجا که در برخی از مطالعات استفاده از متی مازول در دوران بارداری با عوارضی توام بوده است برخی از صاحب نظران استفاده از پروپیل تیوراسیل را در دوران بارداری توصیه می کنند. ید رادیواکتیو برای درمان پرکاری تیرویید در دوران بارداری نباید تجویز شود. در صورتی که درمان دارویی موثر نباشد تیروییدکتومی در سه ماهه دوم بارداری راه دیگری برای کنترل پرکاری تیرویید در مادر باردار است. متی مازول داروی اصلی در دوران شیردهی است.
نتیجه گیری: تشخیص و درمان پرکاری تیرویید در دوران بارداری و شیردهی از اهمیت خاصی برخوردار است. عدم تشخیص و درمان مناسب این اختلال عوارض مهمی را برای مادر و جنین به همراه خواهد داشت.
Background and Aim: Poorly treated or untreated maternal overt hyperthyroidism may affect pregnancy outcome. Thyroid dysfunction and hyperthyroidism is present in about 0.3% and 2-3% of pregnancies respectively. Thyroid hyperfunction and hypofunction are more prevalent and usually go unrecognized. If thyroid dysfunction remained undiagnosed and has not been treated appropriately، would result in serious adverse pregnancy outcomes and treat both mother and her fetus. The aim of this review is to mention many aspects of hyperthyroidism in pregnancy and lactation in depth.
Materials and Methods: Literature review was performed using MEDLINE between years 1960 and 2010، with the terms “Hyperthyroidism and pregnancy”، “Anti-thyroid drug and pregnancy”، “Radioiodine and pregnancy”، “Hyperthyroidism and lactation”، “Anti-thyroid drug and lactation ”، both separately and in conjunction with the terms “fetus”، “neonate” and “maternal”. We selected Proper study design of survey، case control and cohort studies، and clinical trials and review papers if the authors had at least four articles of their own in the list of references of review paper. The strategy used to search for articles was developed with the assistance of a research librarian.
Results: Antithyroid drugs are the main therapy of maternal hyperthyroidism during the lactation. All forms of antithyroid drugs can be used in pregnancy. As there are some reports regarding teratogenicity of methimazole (MMI)، Propylthiouracil (PTU) is preferred in the first trimester and should be replaced by MMI after this trimester. Radioiodine is absolutely contraindicated for treatment of hyperthyroidism in pregnancy. Subtotal thyroidectomy in second trimester is indicated if hyperthyroidism is uncontrolled. MMI is the mainstay of the treatment of postpartum hyperthyroidism، in particular during lactation.
Conclusion: Management of hyperthyroidism during pregnancy requires special considerations because maternal thyroid disease could have adverse effects on the mother، fetus and neonate.
