چکیده:
سابقه و هدف: آپاندیسیت، یکی از متداول ترین ضایعات التهابی دستگاه گوارش است که افراد زیادی به آن مبتلا می شوند. عوامل ایجاد کننده ی آپاندیسیت متعدد می باشد. در میان این عوامل، عفونت های باکتریایی دستگاه گوارش، سهم عمده ای دارند. از مهمترین عوامل باکتریایی ایجاد کنندهی آپاندیسیت، کمپیلوباکتر می باشد. در این مقاله، شش مورد پسودوآپاندیسیت ناشی از گونه های کمپیلوباکتر گزارش شده است.
مواد و روش ها: طی یکسال، از 216 نمونه ی بافتی آپاندیسیت، با استفاده از روش غنی سازی در آبگوشت تیوگلیکولات (5/8pH=) اقدام به جداسازی کمپیلوباکتر گردید. پس از 24 ساعت، نمونه ها در محیط charcoal blood حاوی آنتی بیوتیک های cefoperazone و amphotericin B (Biomerieux) کشت داده شدند. پلیت ها در شرایط میکروآیروفیلیک در دمای 42 درجه سانتی گراد، به مدت 48 ساعت انکوبه شدند. گونه های کمپیلوباکتر با شکل مشخص S در رنگ آمیزی گرم، متحرک، اکسیداز مثبت و هیدرولیز هیپورات، انتخاب شدند.
یافته ها: از 216 نمونه آپاندیسیت مورد بررسی، 5 مورد کمپیلوباکتر ژژونی و 1 مورد کمپیلوباکتر فتوس جدا گردید. ایزوله های کمپیلوباکتر، مقاومت بالایی نسبت به برخی از آنتی بیوتیک ها نظیر تتراسایکلین و نالیدیکسیک اسید، از خود نشان دادند.
نتیجه گیری: نتایج بدست آمده نشان می دهد در موارد مشکوک به آپاندیسیت، نیاز به توجه بیشتر به عوامل میکروبی، به ویژه گونه های کمپیلوباکتر وجود دارد.
Background and Aim: Appendicitis is inflammation of the appendix and is a medical emergency، that many people could be infected. Many factors among them causes appendicitis. Bacterial infection of gastrointestinal tract plays a major role and Campylobacter is the most important in causing appendicitis. The aim of this study was to investigate the prevalence of Campylobacter in suspected samples of appendicitis.
Materials and Methods: This is a descriptive study. In a period of one year، 216 samples of appendicitis tissues were investigated. The samples first cultured in enrichment medium (thioglycolate pH=8.5) for 24 hours، and then were cultured in charcoal blood containing cefoperazone and amphotericin-B (Biomerieux) antibiotics. The cultures were incubated at microaerophilic conditions at 42 C for 48 hours. The Campylobacter stains appeared S shaped on culture. The gram stain، motility، oxidase، and hippurate hydrolysis were carried out on the colonies for the identifications.
Results: Out of 216 tested samples، five were identified as Campylobacter jejuni، and one as Campylobacter coli. The resistance antibiotic patterns showed high level of resistance to tetracycline and nalidixic acid.
Conclusion: The results of this study showed that in case of suspected appendicitis، more attention should be paid to microbial cause especially Campylobacter strains.
