چکیده:
زمینه و هدف: در یک زانوی پرانتزی تعادل بار در دو طرف مفصل زانو بههمخورده و تنش وارده به قسمت داخلی مفصل بیشتر از مقدار آن در مقایسه با یک پای سالم است. معمولترین نوع جراحی برای ایجاد تعادل در بار مفصل، جراحی موسوم به استئوتومی قسمت بالای تیبیا است. روش بررسی: در این مطالعه، مدل سهبعدی برای یک نمونهی مفصل زانوی پرانتزی از روی تصاویر امآرآی تولید شده است. سپس مدل نمونهی دوم برای همان مفصل بعد از انجام عمل مجازی استئوتومی گوه بسته، تولید شده است. برای وضعیت ایستادن معمولی، پس از بارگذاری و حل مسأله، وضعیت توزیع تنش در منیسک و غضروفهای مفصل با استفاده از روش اجزای محدود برای هر دو حالت بهدست آمده است. یافتهها: نتایج نشان میدهد که مقادیر حداکثر تنش فون میسز (حداکثر اعوجاج) و نیز فشار تماسی برای زانوی پرانتزی مقادیر بسیار بالاتری نسبت به پای عمل شده دارد. حداکثر فشار تماسی، تحت یک بار 400 نیوتنی که بر انتهای بالایی سر استخوان فمور وارد شده، برای زانوی پرانتزی قبل از عمل 821/7 مگاپاسکال در منیسک داخلی و برای زانوی عمل شده 099/4 مگاپاسکال و در منیسک خارجی بهدست آمد. همچنین حداکثر تنش فون میسز برای قبل و بعد عمل به ترتیب 501/6 و 488/3 مگاپاسکال بود که هر دو در منیسک داخلی دیده شدند. نتیجهگیری: پس از شبیهسازی و تحلیل اجزای محدود، نتایج توزیع تنش نشان داد که تنشها پس از عمل، توزیع مناسبتری پیدا میکنند و به مقادیر یک پای سالم نزدیکتر میشوند.
Background and aim: In a varus knee, the load balance on two sides of the knee joint is disturbed and the stress applied to the medial side of the joint will be greater than that of a healthy knee. The most common type of surgery to correct the deformity and balance the joint load is surgery called High Tibial Osteotomy. Materials and method: In this study, a 3D model for a varus knee joint specimen was generated from MRI images. The second sample 3D model is prepared for the same joint after performing a virtual Closed Wedge Osteotomy. For the normal standing position, after loading and solving the problem, the stress distribution in the meniscus and the articular cartilage for both samples is obtained. Results: The results show that the maximum values of Von Mises stress and Contact Pressure for the varus knee are much higher than the corresponding values for the operated knee. The maximum contact pressure, under a 400 Newtons load applied to the upper end of the femoral head, was 7.821 MPa in the internal meniscus for the preoperative knee and 4.099 MPa for the operated knee in the external meniscus. Also, the maximum von Mises stress for before and after surgery was 6.501 and 3.488 MPa, respectively, both in the inner meniscus.Conclusion: After simulation and finite elements analysis, the results of stress distribution showed that the stresses find a more suitable distribution after the operation and are closer to the values of a healthy knee.
