Abstract:
زمینه : قطع اندامهای تحتانی در هر سطحی، فرد را مستعد بروز یکسری آسیب ها و عوارض ثانویه در دستگاه عضلانی – اسکلتی می کند که بسته به سطح قطع، هم عضو باقیمانده اندامهای تحتانی و هم اندامهای فوقانی را درگیر می کند. از طرفی بسته به اینکه فرد از وسایل کمکی استفاده می کند یا نه، شیوع این عوارض متغیر می باشد.
هدف : چون تحمل وزن انسان از طریق اندام های تحتانی صورت می گیرد و وجود اندامهای تحتانی برای حرکت و عملکرد کلی بدن ضروری است، لذا با افزایش شیوع عوارض ناشی از قطع اندام، کارآیی این افراد نیز کاهش می یابد، پس اطلاعات مربوط به شیوع عوارض، توانایی پیشگیری را افزایش می دهد و کارآیی و استقلال را بالا می برد. نوع مطالعه ما از نوع مقطعی (Cross Sectional) و نمونهگیری از نوع ساده (Convenient) میباشددر این مطالعه 335 نفر شرکت کردند که 327 نفر مرد و 8 نفر زن بودند. 97% از این جانبازان، درصد جانبازی 70% داشتند. میانگین سنی آنها ( 6.3 SD=)42 بود. این جانبازان از 26 استان دعوت شده بودند وعوارضی نظیر درد در اندامهی فوقانی و باقیمانده اندام تحتانی تادونیتها وانحرافات ستون فقرات مورد ارزیابی قرار گرفت .
نتیجه :بررسیهای ما نشان داد که که درد زانو (چپ8.1% و راست7.5%)، شایع ترین علت درد مفصلی در اندام های تحتانی و درد شانه (9% راست و9.6% چپ)، شایع ترین علت درد مفصلی در اندام های فوقانی جانبازان دو پا قطع میباشد و بیشتر جانبازان (5/52 درصد) درد مهره های لومبوساکرال دارند شایع ترین تاندونیت ها در اندام های فوقانی به ترتیب تاندونیت عضله بای سپس (راست4.8% و چپ4.2% ) و تاندونیت خارجی آرنج (راست3.3% و چپ3.3%) و بورسیت ایسکیال (3% راست و3.3% چپ)، شایع ترین بورسیت اندام تحتانی در جانبازان دوپا قطع میباشد.
بحث : اطلاعات مربوط به شیوع عوارض، توانایی پیشگیری را افزایش می دهد و کارآیی و استقلال را بالا می برد همچنین با اطلاع دقیق از این عوارض عوامل ارتوپدی فنی با دید بهتر و علمی تر به نقایص کارخود پی برده و تلاش خود را جهت ارتقای فنی و عملکردی محصولاتشان افزایش می دهند
Background: Bilateral lower limb amputation in any level causes outbreak of some injuries and secondary side-effects in musculoskeletal system and depending on the level of dissection it involves lower limb stump including the upper limbs. Furthermore, the incidence of these side-effects varies depending on using any accessory equipment.
Purpose :While lower extremities carry all the weight of body and consequently, the presence of lower limbs is crucial for movement and the general performance of body, the increase in the incidence of side-effects originated from amputation lowers the abilities of the person. Therefore, the information about the incidence of side-effects increases the ability of prevention and subsequently, Functional Independence Measure.
Method & materials :Our study is a Cross Sectional type and the sampling was based on the convenient method. In this study, 335 people including 327 men and 8 women were chosen and 97% of them had more than 70% of disability. The average age was 42 6/3. They were invited from 26 provinces and several side-effects such as pain in upper limbs and lower stump , spinal deformity and tendinitis were evaluated.
Results:The investigations showed that knee pain (left: 8.1%, right: 7.5%) were the most incident pain in lower limb and shoulder pain (left: 9.6%, right: 9%) were the most incident juncture pain in upper limbs in disabled people with both lower limb amputation . Moreover, most of the disabled (52.5%) suffered from pain in lumbosacral vertebra. The most incident tendinitis in upper limbs were biceps tendinitis (right: 4.8%, left: 4.2%) and lateral epicondylitis (right: 3.3%, left: 3.3%) respectively and Ischial bursitis (right: 3%, left: 3.3%) was the most incident bursa in lower limbs.
Conclusion:The information about the incidence of side-effects increases the ability of prevention and elevates Functional Independence Measure. In addition, the technical orthopedist will have better and more scientific view of their drawbacks and problems by employing the accurate knowledge they have about these side-effects and will try for improve technical and functional performance of their products.
