Abstract:
هدف پژوهش حاضر تبیین نقش LVEF، ادراکات بیماری و عواطف مثبت و منفی در کیفیت زندگی مرتبط با سلامت (HRQOL) بیماران CAD و طراحی یک مدل مفروض بر اساس این متغیرهابود. در یک بررسی توصیفی -همبستگی ، ۲۷۰ بیمار مبتلا به CAD از بیمارستان مرکز قلب تهران به روش نمونه گیری در دسترس انتخاب شدند. برای جمع آوری داده ها از فرم کوتاه پرسشنامه ادراک بیماری (Brief IPQ)، فرم کوتاه مقیاس بین المللی عاطفه مثبت منفی (I-PANAS-SF) و پرسشنامه آنژین سیاتل (SAQ) استفاده شد. همچنین ، دستگاه اکوکاردیوگرافی برای به دست آوردن LVEF مورد استفاده قرار گرفت . شاخص های برازندگی و نتایج معادلات ساختاری از مدل مفروض در خصوص اثرات LVEF، ادراکات بیماری و عواطف مثبت و منفی بر کیفیت زندگی مرتبط با سلامت بیماران CAD حمایت کرد و نشان داد که این مدل از برازش مطلوب برخوردار است . اثر مستقیم LVEF بر کیفیت زندگی مرتبط با سلامت نیرومندتر از اثر غیرمستقیم آن از طریق ادراک بیماری است . همچنین اثر مستقیم و غیرمستقیم آن از طریق عواطف مثبت و منفی بر کیفیت زندگی مرتبط با سلامت معنی دار نیست . این یافته ها نقش اساسی LVEF، ادراکات بیماری و عواطف مثبت و منفی در کیفیت زندگی مرتبط با سلامت بیماران CAD را نشان داد. بنابراین ، لازم است مداخلاتی جهت افزایش سطوح LVEF و مداخلات متمرکز بر ادراکات بیماری و عواطف مثبت برای این بیماران تدارک دیده شود.
The objective of this study was to explain the role of LVEF، illness perceptions، and positive/negative affect in health related quality of life (HRQOL) in patients with CAD and was to design a hypothesized model based on these variables. This study was conducted with a correlational method، in which 270 patients with CAD from Tehran Heart Center Hospital were selected by convenience sampling method and asked to answer Positive and Negative Affect Schedule (PANAS) Short Form (I-PANAS-SF)، Brief Illness Perception Questionnaire (Brief IPQ) and Seattle Angina Questionnaire (SAQ). Data for LVEF of patients was obtained by echocardiography. The results of Structural Equation Modeling confirmed the hypothesized model regarding the effect of LVEF، illness perceptions and positive/negative affect on health related quality of life in patients with CAD. Besides، it showed that this model had an appropriate fitness. Direct effect of LVEF on HRQOL was stronger than it’s indirect effect through illness perception. Also، the direct and indirect effects of LVEF on HRQOL through positive/negative affect was not significant. These results suggest that LVEF، illness perceptions and positive/negative affect had an essential role in health related quality of life in patients with CAD. Therefore، it is necessary to provide interventions to increase LVEF and interventions focused on illness perceptions and positive/negative affect for these patients.

براساس این مدل می توان گفت که LVEF، ادراک بیماری و عواطف مثبت و منفی ، کیفیت زندگی بیماران مبتلا به CAD را تحت تأثیر قرار می دهد، بنابراین لازم است که مداخلاتی جسمانی برای بهبود LVEF بیماران و مداخلاتی روان شناختی متمرکز بر ادراک بیماری برای تغییر باورهای بیمار در خصوص بیماری و مداخلاتی جهت گسترش عواطف مثبت در بیمار تدارک دیده شود.
