Abstract:
زمینه و هدف: از آنجاییکه ورزشکاران بعد از پارگی رباط صلیبی قدامی خستگی را تجربه میکنند، بنابراین شناخت اثرات خستگی بر کنترل عصبی عضلانی و اجرای عملکردی حائز اهمیت است. هدف تحقیق حاضر بررسی اثر خستگی بر میزان هم انقباضی عضلات اطراف زانو و اجرای عملکردی در ورزشکاران با سابقه ی پارگی رباط صلیبی قدامی بوده است. روش بررسی: نمونه تحقیق حاضر شامل 36 ورزشکار بودند که در سه گروه ،12 نفر با عمل بازسازی رباط صلیبی (ACLR)، 12 نفر با پارگی رباط صلیبی بدون عمل بازسازی (ACLD) و 12 نفر به عنوان گروه کنترل قرار گرفتند. میزان هم انقباضی عضلات چهارسر-همسترینگ، چهارسر-دوقلو و اجرای عملکردی در قبل و بعد از خستگی مورد ارزیابی قرار گرفت. یافته ها: میزان هم انقباضی عضلات چهارسرران-دوقلو خارجی در گروه های آسیب دیده (ACLR و ACLD) به طور معناداری کمتر از گروه کنترل بود. همچنین اجرای عملکردی در تست لی تک پا در گروه های آسیب دیده به طور معناداری بعد از خستگی کاهش یافت. نتیجه گیری: هم انقباضی عضلات چهارسر-دوقلو در ایجاد ثبات مفصل زانو مهم است، لذا توصیه می شود در تمرینات توانبخشی این دو عضله بیشتر مدنظر قرار گیرد. با توجه به کاهش اجرای عملکردی در ورزشکاران آسیب دیده ، پیشنهاد می شود آزمون های عملکردی در شرایط خستگی نیز ارزیابی شوند.
Background and aim: As athletes returning to activity after anterior cruciate ligament injury likely experience fatigue, understanding how fatigue affects neuromuscular control and functional performance has the clinical importance. The purpose of this study was to determine the effect of fatigue on knee muscles co-contraction and functional performance of the athletes with the history of anterior cruciate ligament rupture. Materials and Methods: Thirty six athletes including 12 individuals with anterior cruciate ligament reconstruction (ACLR), 12 individuals with the anterior cruciate ligament rupture without reconstruction (ACLD) and 12 controls were recruited to participate in the study. Quadriceps to hamstrings, quadriceps to gastrocnemius co-contraction and functional performance were measured before and after fatigue. Results: Lateral quadriceps to gastrocnemius co-contraction was significantly lower in ACL injured groups compared to controls. Also, functional performance in single leg hop test significantly decreased in ACL injured groups post fatigue. Conclusion: It seems that quadriceps and gastrocnemius muscle co-contraction have important role in establishing knee joint stability, so these two muscles should be more considered in rehabilitation exercises. Also, for a more comprehensive evaluation of lower extremity function after ACL injury, it is therefore suggested that functional testing should be performed both under non-fatigued and fatigued test conditions.
