Abstract:
اهداف: تاثیر روشها و داروهای بیهوشی بر سیستمها و ارگانهای مختلف بدن به ویژه سیستم ایمنی در بیماران سالمند تحت عمل جراحی، همواره مورد توجه محققین بوده و در این مطالعه اثرات ناشی از بیهوشی عمومی با داروی ایزوفلوران و بیحسی نخاعی با لیدوکائین بر شاخصههای اکسیدانی و آنتی اکسیدانی مورد بررسی قرا گرفت.
مواد و روشها: این مطالعه کارآزمایی بالینی دوسوکور با طراح موازی بر روی بیماران سالمند مراجعه کننده به بخش جراحی در طی سالهای 1399 -1398 انجام گرفت. بیماران شرکت کننده به صورت کاملا تصادفی به دو گروه 35 نفره تقسیم شدند. در یک گروه برای بیهوشی از گاز بیهوش کننده ایزوفلوران و در گروه دیگر برای بیهوشی از بیحسی نخاعی با لیدوکائین استفاده گردید. جمع اوری نمونه در این مطالعه در دو مرحله یک روز قبل (نمونه کنترل) و یک روز بعد از عمل جراحی (نمونه آزمون) صورت گرفت. در این مطالعه شاخصهای AGEs، AOPP، MDA، oxLDL، FRAP، گلوتاتیون پراکسیداز، سوپراکسید دیسموتاز و کاتالاز به عنوان شاخصهای سیستم ایمنی در طی قبل و بعد ازانجام بیهوشی یا بیحسی نخاعی و توسط روشهای استاندارد اندازهگیری شد. در این مطالعه از نرم افزار آماری SPSS 20 برای تجزیه و تحلیل دادهها استفاده شد.
یافته ها: میانگین سنی بیماران در گروه لیدوکایین (8/31 ± 69/74) و ایزوفلوران (4/98±70/23) اختلاف معنی داری با یگدیگر نداشت (P=0/81). در این مطالعه، شاخصهای Advanced Glycation End Products، Malonaldehyde، oxidized LDL، Ferric Reducing Ability of Plasma، Catalase، Glutathione Peroxidase و Superoxide Dismutase در بیماران تحت بیهوشی با ایزوفلوران، اختلاف معنی داری بین قبل و بعد از عمل داشتند در خصوص بیماران بیهوش شده با لیدوکایین، این تفاوت در خصوص شاخصهای Malonaldehyde، oxidized LDL، Catalase و Superoxide Dismutase مشاهده شد.
نتیجهگیری: با توجه به تاثیرات مشاهده شده گاز بیهوشی ایزوفلورن بر روی شاخصهای اکسیدانی و آنتی اکسیدانی در بیماران سالمند، پیشنهاد میشود که این دارو در انتخاب روشو داروی بیهوشی برای سالمندان تحت عمل جراحی مورد توجه قرار گیرد.
Objective: The effects of anesthesia thechniques and drugs on different body systems and organs, especially immune system in elderly patients who underwent operation, have always been investigated by investigators The aim of this study was to compare the effects of general anesthesia with isoflurane or spinal anesthesia with lidocaine on oxidative and antioxidant indices.
Materials & Methods: In this study, double-blind randomized clinical trial with parallel design was performed on elderly patients who underwent operation between 2018 and 2019. Patients were randomly divided into two groups (n = 35). In the first group patients received isoflurane gas and in the other group received lidocaine spinal anesthesia. Study sampling was performed in two stages; one day before (control sample) and one day after operation. In this study, AGEs, AOPP, MDA, LDL, FRAP, glutathione peroxidase, superoxide dismutase and catalase were measured.
Results: In this study and among patients of isoflorane group pre and postoperative measurement of Advanced Glycation End Products, Malonaldehyde, oxidized LDL, Ferric Reducing Ability of Plasma, Catalase, Glutathione Peroxidase and Superoxide Dismutase had significant difference. For patients in the lidocaine group, this difference was observed for the indices including Malonaldehyde, oxidized LDL, Catalase and Superoxide Dismutase.
Conclusion: With regards to positive effects of anesthetic gases such as isflorane on oxidative and antioxidant indices in elderly patients, these results should be considered in the selection of anesthesia methods and drugs for the elderly patient underwent operation.
