Abstract:
زمینه: با توجه به تغییرات در DSM و در نظر گرفتن رویکرد ابعادی به جای رویکرد طبقاتی این سؤال مطرح است که آیا درمان فراتشخیصی بر درمان اختلال اختلال وسواس- ناخودداری مؤثر است؟ هدف: این پژوهش با هدف بررسی اثربخشی درمان فراتشخیصی بر بهبود، تحمل ابهام، کارکرد اجرایی (دقت و زمان) و حافظه در بیماران با اختلال وسواس- ناخودداری انجام شد. روش: از میان افراد مبتلا به اختلال وسواس- ناخودداری مراجعه کننده به مراکز خدمات روانشناختی قزوین 40 بیمار با روش نمونه گیری هدفمند انتخاب و به طور تصادفی در دو گروه 20 نفری (گروه تحت درمان فراتشخیصی و گروه گواه) قرار گرفتند و پرسشنامه های وسواس ییل براون(گودمن، پریس و همکاران، 1989)، مقیاس تحمل ابهام )مکلین، ،2009) ، آزمون فراخنای حروف و ارقام (وکسلر، 1987) و آزمون استروپ (استروپ، 1935) را تکمیل کردند. یافته ها: نتایج به دست آمده نشان داد که درمان فراتشخیصی به طور معناداری باعث بهبود تحمل ابهام، کارکرد اجرایی (دقت و زمان) و حافظه در افراد مبتلا به اختلال وسواس-ناخودداری شده است (0/001>p). نتیجه گیری: رویکرد فراتشخیصی در درمان با مدنظر قرار دادن رویکرد ابعادی و هدف قرار دادن سازه های فراتشخیصی می تواند در بهبود علایم OCD اثرگذار باشد.
Background: Given the changes in DSM and the consideration of dimensional variation, instead of category approach, the question arises as to whether transdiagnostic therapy is effective in the treatment of OCD? Aims: The purpose of this study was to investigate the effectiveness of transdiagnostic treatment on ambiguity tolerance, executive functions (accuracy and time) and memory in patients with obsessive-compulsive disorder (OCD). Method: 40 patients were selected by purposeful sampling method and randomly assigned to 2 groups of 20 subjects (transdiagnostic treatment group and control group). The tools used in the present study were: Yale Brown Obsessive Compulsory Inventory )Goodman, Price et al. 1989(, McLean ambiguity tolerance scale)McLain 2009), and Letter–Number Sequencing(Wechsler,1987) and Stroop test )Stroop,1935). Results: The results showed that transdiagnostic treatment significantly improved ambiguity tolerance, cognitive emotion regulation, executive functions (accuracy and time) and memory in patients with OCD (pConclusion: transdiagnostic approach in treatment by considering the dimensional approach and targeting of diagnostic structures can be effective in improving the symptoms of OCD.
