Abstract:
مقدمه
در زمینه پیامدهای اقتصادی برون سپاری خدمات در بیمارستان های دولتی ابهام ها و تناقض های زیادی وجود دارد. ازاین رو، مطالعه حاضر با هدف بررسی پیامدهای اقتصادی برون سپاری در بیمارستان های دولتی در ایران انجام شد.
روش ها
این مطالعه به صورت مرور نظام یافته با جستجوی کلیدواژه های مرتبط در پایگاه های اطلاعاتی داخلی و خارجی PubMed، ProQuest، Scopus،Web of Science، SID، MagIran و موتور جستجوی Google Scholar بدون محدودیت زمانی انجام شد. پس از حذف موارد تکراری و ارزیابی بر اساس معیار ورود و خروج پژوهش، مطالعات مرتبط انتخاب شدند.
یافته ها
از ۸۴۰ مقاله بازیابی شده در نهایت ۲۰ مقاله وارد مطالعه شد. سه نوع برون سپاری اجاره ای، مدیریتی و مشارکت دولتی-خصوصی شناسایی شد. در پنج مطالعه کل بیمارستان برون سپاری شده بود. هیچ یک از مطالعات کمی دارای گروه کنترل نبودند. در کل از ۵۱ شاخص استفاده شده بود که بیشترین آن ها مربوط به درآمد کل، هزینه های کل و هزینه های پرسنلی بود. میانگین زمان بررسی اثرات برون سپاری ۲/۲۴ ماه بود. اکثر مطالعات بیانگر تاثیرات مثبت برون سپاری از جنبه های اقتصادی بود. مهم ترین دستاوردهای برون سپاری در این پژوهش می توان به کاهش هزینه و افزایش سودآوری و مهم ترین موانع به وجود هزینه های پنهان، تحمیل هزینه های اضافی به بیماران، نبود ثبات اقتصادی و ارزیابی نکردن از لحاظ مالی اشاره کرد.
نتیجه گیری:
نتایج نشان داد که برون سپاری در بسیاری از بخش ها و خدمات بیمارستان های دولتی موفقیت هایی بسیاری از نظر اقتصادی داشته است. اما در برخی از موارد برون سپاری ها سبب افزایش هزینه ها شده است. بنابراین، بررسی وسیع تر جهت شناسایی عوامل تاثیرگذار و پیامدهای منفی برون سپاری و راهکارهای مقابله با آن باید موردتوجه مدیران بیمارستان و سیاست گذاران سلامت قرار گیرد.
Introduction
There are many ambiguities and contradictions in the economic consequences of outsourcing services in public hospitals. Therefore, the aim of the present study was to systematically review the economic consequences of outsourcing in public hospitals in Iran.
Methods
The information required in this systematic review study was collected using relevant keyword searches in the databases of PubMed, ProQuest, Scopus, Web of Knowledge, Google Scholar, SID, and MagIran without time limit. Textual data were analyzed manually by the content analysis method.
Results
Finally, 20 articles were included in the study. Three types of rental outsourcing, managerial, and public-private partnership were reported in the studies. In five studies, the whole hospital was outsourced. None of the studies had a control group. A total of 51 indicators were used, most of which were total revenues, total costs, and personnel costs. The mean study time of the outsourcing effects was 24. 2 months. The results of most studies showed the positive effects of outsourcing in economic terms. The most important achievements of outsourcing included reducing costs and increasing profitability, and the most important obstacles included the existence of hidden costs, imposing additional costs on patients, lack of economic stability, and lack of economic evaluation.
Conclusion
The results showed that outsourcing has been economically successful in many wards and services of public hospitals; however, in some cases, outsourcing has led to increased costs. Therefore, more attention should be paid by hospital managers and health policy makers to identify the influencing factors and negative consequences of outsourcing.
