Abstract:
Previous studies have revealed that many professionals are willing to use spiritual
interventions in clinical settings; however, they have access to no standard guide for
their implementation, evaluation, and upgrading. This study aimed to determine the
role of the Spiritual Intervention Package (SIP) in altering the expression of serotonin
and dopamine gene receptors, the serum levels of interleukin 10-interferon gamma
and the necrosis factor of the alpha tumor, and blood cortisol levels, promoting
general health, decreasing distress and sexual problems, and reinforcing empathy,
marital satisfaction, and posttraumatic growth. To this end, this multi-axial research
consisting of 10 independent studies (16-60 persons per group) was conducted and
examined the effectiveness of SIP in a variety of clinical settings among women with
breast cancer stages 1-3, who were visited at Shohadaye Tajrish Hospital during
2012-2017. The participants were selected using the purposive and voluntary
sampling methods. The levels of IL4 and IFNγ, the percentage of T-CD, and 8 T-CD
cells were assessed by flow cytometer. Moreover, general health, somatization,
anxiety, social dysfunction, and depression; spiritual health, sexual problems, marital
satisfaction, and posttraumatic growth were evaluated using standard questionnaires.
The research showed the acceptable results for the implementation of SIP for
concerned purposes. The effect size evaluation– a technique to compare the impacts
of the SIP regardless of the sample size, and the squared ETA (R2) assessment
showed that, on average, more than 70% of the observed changes in these studies
were due to the application of spirituality intervention. The effect size for biological,
psychological, and social measures revealed that SIP significantly improved the
biological, psychological, and social status of patients with breast cancer.
مطالعات قبلی نشان داده است که بسیاری از متخصصان مایل به استفاده از مداخلات معنوی در محیط های بالینی هستند. با این حال، هیچ راهنمای استانداردی برای اجرا، ارزیابی و ارتقاء خود دسترسی ندارند. این مطالعه با هدف تعیین نقش بسته مداخله معنوی (SIP) در تغییر بیان گیرندههای ژن سروتونین و دوپامین، سطوح سرمی اینترلوکین 10-اینترفرون گاما و فاکتور نکروز تومور الفا، کورتیزول خون، سلامت عمومی، کاهش پریشانی و مشکلات جنسی، تقویت همدلی، رضایت زناشویی و رشد پس از سانحه انجام شد. برای این منظور، طی یک تحقیق چند محوری شامل 10 مطالعه مستقل (16-60 نفر در هر گروه) اثربخشی SIP در انواع شرایط بالینی در بین زنان مبتلا به سرطان پستان در مراحل 1-3 مورد بررسی قرار گرفت. شرکت کنندگان با استفاده از روش نمونه گیری هدفمند و داوطلبانه از بین بیماران بیمارستان شهدای تجریش طی سال های 1391 تا 1396انتخاب شدند. سطوح IL4 و IFNγ، درصد T-CD و 8 سلول T-CD توسط فلوسایتومتر ارزیابی شد. علاوه بر این، سلامت عمومی، جسمانی شدن، اضطراب، اختلال عملکرد اجتماعی و افسردگی. سلامت معنوی، مشکلات جنسی، رضایت زناشویی و رشد پس از سانحه با استفاده از پرسشنامه استاندارد ارزیابی شد. این تحقیق نتایج قابل قبولی را برای اجرای SIP برای اهداف مورد نظر نشان داد. ارزیابی اندازه اثر برای مقایسه تاثیرات SIP بدون توجه به حجم نمونه، و ارزیابی مجذور ETA (R2) نشان داد که به طور متوسط بیش از 70 درصد از تغییرات مشاهده شده در این مطالعات به دلیل کاربرد مداخله معنوی بوده است. به نظر می رسد مداخله معنوی وضعیتهای زیستی، روانی و اجتماعی بیماران مبتلا به سرطان پستان را بهبود می بخشد.

Results The present study was to investigate the impact of the spiritual intervention on the improvement of biological (namely gene expression, cortisol, and cellular immune response), psychological (namely general health, distress, sexual problems, psychological well-being), social (namely empathy, marital satisfaction) and spiritual (namely spiritual well-being, posttraumatic development) dimensions in women with breast cancer.
