Abstract:
با وجود اینکه رشد و تکامل سیستم ایمنی و دفاعی از هفته پنجم دوره جنینی آغاز میشود، اما در هنگام تولد هنوز تکامل نیافته است و در برخی موارد هنوز عملکرد مناسبی از خود نشان نمیدهد. به عنوان مثال سلولهای بیگانه خوار طی شش ماه اول عمر نوزاد از نظر عملکردی تکامل مییابند. تعداد کمی از سلولهای T به شکل بالغ و کار آمد به عنوان فرمانده سیستم ایمنی انجام وظیفه مینمایند. سلولهای B به عنوان سلولهای مولد آنتی بادی از کارایی ضعیفی برخوردارند و بعضی از زیرکلاسهای آنتی بادی تا دو سالگی هنوز ساخته نمیشوند. تکامل و شکلگیری برخی از سلولهای B در نواحی سرحدی طحال تا دو سالگی به طول میانجامد. این سلولها نقش عمدهای در پاسخهای آنتی بادی و ایمنی اولیه به عوامل بیماریزا دارند به خصوص عواملی که از راه خون وارد بدن میشوند. تا شکلگیری نهایی سیستم ایمنی، نوزاد در خطر ابتلا به بسیاری از عوامل و بیماریهای عفونی است. شیر مادرعلاوه برنقش تغذیهای به عنوان بخشی از سیستم ایمنی نوزاد نیز عمل میکند. شیر مادر دارای عوامل ایمنی ذاتی نظیر پروبیوتیکها و پپتیدهای ضد میکروبی است و واجد عوامل سیستم ایمنی اکتسابی نظیر لکوسیتها و آنتی بادی از کلاس IgA است و از این طریق به نوزاد در برابر عوامل عفونی مصونیت اعطا مینماید. در هفت آیه از آیات مصحف شریف به بحث در مورد شیر مادر میپردازد. در آیه 233 سوره بقره که مشهورترین آیه در خصوص شیر مادر است به عبارت "حولین کاملین" برمی خوریم که اشاره به دوره شیردهی کامل (حداکثر دوران مفید شیردهی) یعنی دو سال تمام دارد. به نظر میرسد با توجه به عدم تکامل سیستم ایمنی و عدم وجود سلولهای کارآمد دفاعی نوزاد تا سن دو سالگی، نوزاد تا رسیدن به این سن بیشتر در معرض ابتلا به بیماریهای عفونی باشد لذا تغذیه با شیر مادر و انتقال عوامل سیستم ایمنی مادر به سیستم ایمنی نوزاد کمک مینماید تا بتواند بیشتر و بهتر در مقابل عوامل عفونی مقابله نماید و او را در مقابل این بیماریها مصون دارد. به عنوان جمع بندی، شاید یکی از دلایلی که قرآن کریم به شیردهی کامل توصیه میکند؛ عدم توسعه کامل سیستم ایمنی نوزاد در این دوره باشد.
Even though the development of the immune system begins from the fifth week of the embryonic period, the system is still under development at the birth time. For example, the phagocyte cells functionally develop during the first 6 months of neonatal life. Also, A few mature and effective T cells, as the commander of the immune system, circulate in the tissues, B cells have poor efficiency as antibody-producing cells, and some antibody subclasses are not synthesized up to two years of age. Furthermore, the development and formation of some B cells in marginal zone of the spleen last up to two years of age. These cells play a major role in antibody responses and primary defense against pathogens, particularly those agents entering the body via the blood. On the other hand, the neonate is exposed to many pathogens and infectious diseases until the final formation of the immune system. Breast milk, in addition to its nutritional value, plays an important role in the protection of neonate against microorganisms and contains innate immunity factors including probiotics and antimicrobial peptides, as well as acquired immunity factors including leucocytes and the IgA class antibody, thereby granting the neonate immunity against infectious factors. In the Holy Qur'an, seven verses discuss about the breast milk. In the most well-known verse about breast milk (Verse 233 of Sura al-Baqarah), the phrase "Hawlaini Kamilaini (in Arabic: حولین کاملین)" refers to the full breastfeeding period (maximum useful period) i.e., two whole years. Due to the underdeveloped immune system and the absence of efficient defensive cells during the first two years of neonates’ life, they seem to be susceptible to infectious diseases up to this period. Therefore, breastfeeding, by transferring the mother's immune factors to the neonate, supports the neonate's immune system against infectious agents and diseases. In conclusion, perhaps one of the reasons that the Holy Qur'an recommends complete breastfeeding is due to the neonates’ underdeveloped immune system in this period.
