Abstract:
مقدمه: تقاضای القایی عبارت است از ارائه مراقبت یا خدمت غیر ضروری به بیماران و مراجعین نظام سلامت با اعمال قدرت و سفارش افراد متخصص. تقاضای القایی باعث افزایش هزینههای سلامت و کاهش کارایی نظام سلامت میشود. بنابراین، پژوهش حاضر با هدف شناسایی راهکارهای کاهش تقاضای القایی در نظام سلامت انجام شد. روش ها: این مطالعه با روش مرور حیطه ای انجام شد. کلیه مقالات منتشر شده در زمینه راهکارهای کاهش تقاضای القایی در نظام سلامت تا 22 بهمن 1401 خورشیدی در 9 پایگاه داده ای و موتور جستجوگر Google Scholar جستجو و جمعآوری شدند. در نهایت، تعداد 36 مقاله مناسب انتخاب شد. داده ها با روش تحلیل موضوعی با استفاده از نرم افزار MAXQDA10 تحلیل شد. یافته ها: تعداد 44 راهکار برای کاهش تقاضای القایی در نظام سلامت شناسایی شد. این راهکارها در سه سطح کلان (نظام سلامت)، میانی (سازمان های بهداشتی و درمانی) و خرد (ارائهکنندگان و گیرندگان خدمات سلامت) گروهبندی شدند. اجرای نظام ارجاع، به کارگیری راهنماهای بالینی، اصلاح روش پرداخت به ارائهکنندگان خدمات سلامت، تعیین سقف تعداد خدمات برای پزشکان، رعایت اخلاق پزشکی از طرف ارائهکنندگان خدمات سلامت، آموزش ارائهکنندگان و گیرندگان خدمات سلامت و نظارت و کنترل وزارت بهداشت از پرتکرارترین راهکارهای کاهش تقاضای القایی در نظام سلامت در مطالعات قبلی بود. نتیجهگیری: تقاضای القایی برای کالاها و خدمات سلامت، ماهیتی پیچیده دارد و در اثر رفتارهای سیاستگذاران و مدیران سلامت، ارائهکنندگان و دریافتکنندگان خدمات سلامت و در نهایت، پرداختکنندگان هزینه خدمات سلامت ایجاد می شود. بنابراین، راهکارهای کاهش تقاضای القایی باید در سه سطح نظام سلامت، سازمانهای بهداشتی و درمانی و ارائهکنندگان و گیرندگان خدمات سلامت بهطور سیستمی بکار گرفته شوند تا نتایج بهتری داشته باشند.
Introduction: Induced demand is the phenomenon where healthcare providers, especially physicians, stimulate demand for unnecessary healthcare services by exerting their influence on patients. Induced demand increases healthcare expenditure and reduces healthcare system efficiency. Therefore, this study aimed to identify strategies for reducing induced demand in the healthcare system.
Methods: In the present scoping review, all published articles on strategies to reduce induced demand in the healthcare system until February 11, 2023, were searched and collected from nine English and Farsi databases and the Google Scholar search engine. Finally, 36 documents were reviewed and analyzed using MAXQDA software.
Results: Overall, 44 strategies to reduce induced demand in the healthcare system were identified and grouped into three levels: macro (healthcare system), meso (healthcare organizations), and micro (healthcare providers and recipients). The most frequent strategies to reduce induced demand included using an appropriate referral system, following clinical guidelines, adopting fixed and pay-for-performance methods of payment to healthcare providers, setting a ceiling on the number of services physicians can offer, enforcing healthcare providers’ compliance with medical ethics, training healthcare providers and recipients, and monitoring and control.
Conclusion: Induced demand for health services has a complex nature and is influenced by the behavior of policymakers, healthcare providers, payers, and patients. Therefore, strategies to reduce induced demand should be applied at the three levels of healthcare system, healthcare organizations, and healthcare providers and recipients to achieve better results.
