چکیده:
مقدمه: تبعیت از درمان یک استراتژی کلیدی در مدیریت بیماریهای مزمن است که عوامل مختلفی بر آن تأثیرگذار است.
هدف: پژوهش حاضر با هدف بررسی نقش واسطهای امید به زندگی در رابطه درک از بیماری، خودشفقتورزی و حمایت اجتماعی ادراک شده با تبعیت از درمان در بیماران قلبی عروقی انجام شد.
روش: این پژوهش از نوع تحقیقات همبستگی به روش معادلات ساختاری بود. جامعه پژوهش شامل کلیه بیماران قلبی عروقی مراجعهکننده به مرکز تخصصی قلب شهید مدنی تبریز در بهار ١٤٠٠ بودند؛ که تعداد ٢٣٤ نفر از آنها بهصورت نمونههای در دسترس انتخاب شدند. ابزارهای این پژوهش شامل: پرسشنامه درک از بیماری برادبنت، مقیاس خودشفقتورزی ریس، حمایت اجتماعی ادراک شده زیمت، امید اشنایدر و پرسشنامه تبعیت از درمان مدانلو بودند. برای تجزیه و تحلیل دادهها از آزمون همبستگی پیرسون، تحلیل مسیر و آزمون سوبل به کمک نرمافزار SPSS و AMOS نسخه ٢٤ استفاده گردید.
یافتهها: نتایج نشان دادند که درک از بیماری، خودشفقتورزی، حمایت اجتماعی ادراک شده و امید به زندگی بر روی تبعیت از درمان؛ و همچنین درک از بیماری، خودشفقتورزی و حمایت اجتماعی ادراک شده بر روی امید به زندگی تأثیر مستقیم و معنادار داشتهاند (٠/٠٠١>p). در ادامه نتایج آزمون سوبل نشان داد که درک از بیماری (٠/٠٩٥=β)، خودشفقتورزی (٠/٠٨٦=β) و حمایت اجتماعی ادراک شده (٠/٠٨٧=β) بهواسطه امید به زندگی بر روی تبعیت از درمان، تأثیر غیرمستقیم و معنادار داشتهاند (٠/٠١>p).
نتیجهگیری: یافتهها نشان دادند که درک از بیماری، خوشفقتورزی و حمایت اجتماعی ادراک شده بهواسطه امید به زندگی موجب افزایش تبعیت از درمان شدهاند. بر این اساس میتوان برای آموزش و ارتقای سطح امید به زندگی و به تبع آن افزایش میزان تبعیت از درمان در بیماران قلبی عروقی، اهدافی را تعیین نمود.
Introduction: Treatment adherence is an indispensable strategy in the management of chronic diseases and can be influenced by various factors.
Aim: The present study aimed to investigate the mediatory role of life expectancy in the relationship of treatment adherence with illness perception, self-compassion, and perceived social support among patients with cardiovascular diseases.
Method: The present study was correlational and was conducted according to structural equation modeling. The population included all patients with cardiovascular diseases that visited Shahid Madani Heart Center in Tabriz, Iran, during the spring of 2021. Then, 234 patients were selected as the sample using the convenience sampling method. The instruments included Broadbent’s Illness Perception Questionnaire, Raes’s Self-compassion Scales, Zimet’s Multidimensional Scale of Perceived Social Support, Snyder’s Adult Hope Scale, and the Modanloo’s Treatment Adherence Scale. Data analysis was conducted using Pearson’s correlation test, Path Analysis, and Sobel’s test in SPSS-24 and AMOS-24.
Results: The findings showed that illness perception, self-compassion, perceived social support, and life expectancy had a direct and significant impacts on treatment adherence, and illness perception, self-compassion, and perceived social supports had a direct and significant impacts on life expectancy (P
Conclusion: The findings showed that illness perceptions, self-compassion, and perceived social support increased treatment adherence by the mediation of life expectancy. Thus, goals could be set to train and increase life expectancy and – consequently – treatment adherence in patients with cardiovascular diseases.
