Abstract:
زمینه و هدف: اختلال طیف اوتیسم، یک اختلال عصبی تحولی است که در سالهای اولیه کودکی ظاهر میشود و درمان قطعی برای آن وجود ندارد. روشهای مختلفی برای آموزش و توانبخشی این اختلال وجود دارد. بر این اساس پژوهش حاضر با هدف مقایسه اثر تمرین در آب، مکمل یاری ویتامین D، و ترکیب تمرین در آب+مکمل یاری بر رفتارهای کلیشه ای و شاخص توده بدن در پسران مبتلا به اختلال طیف اوتیسم انجام شد.
روش: پژوهش آزمایشی حاضر با استفاده از طرح پیش آزمون-پس آزمون با گروه های تصادفی انجام شد. بدین ترتیب 40 کودک مبتلا به اختلال طیف اوتیسم با میانگین سنی 14/2 ± 9/10 سال از موسسه خیریه انجمن اوتیسم گیلان در سال 1397به روش نمونهگیری در دسترس، انتخاب شده و به تصادف در چهار گروه تمرین در آب (10 نفر)، مکملیاری ویتامین D (10 نفر)، ترکیب تمرین+مکمل (10 نفر)، و گواه (10 نفر) جایدهی شدند. قد، وزن، و شاخص توده بدن کودکان در ابتدا و در پایان پژوهش، اندازه گیری شد. گروه های آزمایشی تمرین در آب و تمرین+مکمل به مدت 10 هفته، هفته ای 2 جلسه 60 دقیقهای به اجرای برنامه تمرین در آب پرداختند. همچنین گروه های مکمل یاری و تمرین+مکمل به مدت 10 هفته، هفته ای یک بار قرص ویتامین D با دوز 50000 IU مصرف کردند. والدین کودکان مورد مطالعه، قبل از شروع آزمایش و پس از پایان 10 هفته مداخله، خرده مقیاس رفتارهای کلیشه ای مربوط به ویرایش دوم مقیاس درجه بندی اوتیسم گیلیام (گارز-2) را تکمیل کردند. در پایان برای تحلیل داده ها از آزمون تحلیل کوواریانس استفاده شد.
یافته ها: نتایج تحلیل داده ها نشان داد که هر سه روش تمرین در آب، مکمل یاری ویتامین D، و ترکیب تمرین+مکمل، تاثیر معنی داری بر نمره خرده مقیاس رفتارهای کلیشه ای و شاخص توده بدن در کودکان اوتیسم داشت (0/05> p)، اما روش تمرین+مکمل، اثر بیشتری نسبت به دو روش دیگر نشان داد.
نتیجهگیری: براساس نتایج پژوهش حاضر میتوان نتیجه گرفت که شنا می تواند درونداد حسی ثابتی فراهم کند و سطح فعالیت جسمانی کودکان با اختلال طیف اوتیسم را بدون تحمل وزن، افزایش دهد. همچنین، با توجه به کمبود قابل توجه ویتامین D در این کودکان، مکمل یاری ویتامین D می تواند موجب بهبود شاخص توده بدن و کاهش شدت علائم اختلال شود.
Background and Purpose: Autism spectrum disorder (ASD) is a neurodevelopmental disorder that occurs in early childhood. There are various methods for training and rehabilitation of this disorder. So, the aim of the present study was to compare the effect of aquatic exercise and vitamin D supplementation on stereotypic behaviors and body mass index (BMI) in boys with ASD.
Method: The present experimental study was conducted using a pretest-posttest design with random groups. Thus, 40 children with ASD (mean aged 10.9±2.14 years) were selected according to convenience sampling and randomly assigned to four groups of aquatic exercise (n=10), vitamin D supplementation (n=10), aquatic+supplementary (n=10) and control (n=10). Height, weight and BMI of children were measured at the beginning and the end of intervention. Aquatic exercise and aquatic+supplementary experimental groups were received the 60-minute water training program for 10 weeks, 2 sessions per week. The Vitamin D supplements and aquatic-supplementary groups also received vitamin D tablets at a dose of 50,000 IU once daily for 10 weeks.
Parents of autistic children completed the subscale of the Gilliam Autism Rating Scale (GARS-2) subscale before the experiment and after the end of the 10-week intervention. Finally, covariance analysis test was used for data analysis.

Conclusion: Based on the results of this study, it can be concluded that swimming can provide a stable sensory input and increase the level of physical activity of children with autism spectrum disorder without weight bearing. Also, given the significant deficiency of vitamin D in these children, vitamin D supplementation can improve body mass index and decrease the severity of symptoms.
