Abstract:
مقدمه: عفونت هلیکوباکترپیلوری شایعترین عفونت در جهان شناخته شده و به طور تقریبی نصف جمعیت جهان به آن دچار میشوند. آلودگی اثر جمع شونده داشته، بیشتر بزرگسالان در دوران کودکی به آن مبتلا میشوند. بر اساس مطالعات اپیدمیولوژیک و حیوانی، کلونیزاسیون دراز مدت هلیکوباکترپیلوری میتواند منجر به آدنوکارسینوم معده و لنفوم مری شود، به خصوص اگر عفونت در اوایل عمر کسب گردد.
مواد و روشها: این تحقیق به روش مقطعی بر روی 458 کودک در گروه سنی 4 ماه تا 15 سال و نمونهگیری به صورت تصادفی در مدارس و مراکز بهداشتی-درمانی شهر سنندج صورت گرفت. ابزار مورد استفاده پرسشنامه و جهت این آزمایش از تست تعیین آنتی ژن هلیکوباکترپیلوری در مدفوع استفاده گردید. سپس از آمار توصیفی و استنباطی، X2 (کای دو)، T-test، آزمون فیشر، آنالیز واریانس یکطرفه برای نشان دادن نتایج استفاده گردید.
یافتهها: در مجموع، میزان شیوع عفونت هلیکوباکترپیلوری در گروه سنی اطفال 2/64% برآورد گردید. کمترین شیوع، مربوط به گروه سنی 5/4-5/3 ماهه (6/43%) بود و این میزان تا سن 15 سالگی به 4/64% افزایش پیدا کرد (01/0> p). میزان شیوع با افزایش تعداد اعضاء خانواده از 3 به 5 نفر بسیار افزایش یابنده بود (005/0> p) و در کودکانی که بیش از 6 ماه از شیر مادر تغذیه شده بودند، میزان شیوع در مقایسه با کودکان تغذیه شده با روشهای دیگر، کمتر بود (002/0> p).
بحث و نتیجهگیری: عفونت هلیکوباکترپیلوری در گروه سنی کودکان در سنندج شیوع بسیار بالایی دارد و با افزایش سن این شیوع افزایش مییابد. آلودگی از همان سنین اولیه کودکی، حتی قبل از شروع تغذیه کمکی شروع میشود و به نظر میرسد، انتقال از راههای دیگر، در کنار انتقال از راه غذا و آب، نقش مهمی ایفا میکند. گر چه شیر یک اثر محافظتی داشته است، ولی نمیتواند جلوی این عفونت را بگیرد.
Introduction: Helicobacter pylori infection is the most common infection in the world and
approximately half of the world's population suffers from it. Pollution has cumulative effect, most
adults get it in childhood. Based on epidemiological and animal studies, long-term Helicobacter
pylori colonization can lead to gastric adenocarcinoma and esophageal lymphoma, especially if
the infection is acquired early in life.
Materials and methods: This cross-sectional research was conducted on 458 children in the age
group of 4 months to 15 years and randomly sampled in schools and health-treatment centers of
Sanandaj city. The instrument used is a questionnaire and for this test, the test for determining
Helicobacter pylori antigen in feces was used. Then, descriptive and inferential statistics, X2 (chi -
square), T-test, Fisher test, one-way analysis of variance were used to show the results.
Findings: In total, the prevalence of Helicobacter pylori infection in the children age group was
estimated at 64.2%. The lowest prevalence was related to the age group of 3.5-4.5 months (43.6%)
and this rate increased to 64.4% until the age of 15 (p < 0.01). The rate of prevalence increased
greatly with the increase in the number of family members from 3 to 5 people (p < 0.005) and in
children who were breastfed for more than 6 months, the prevalence rate was higher compared to
children who were fed with other methods. Others were less (p < 0.002). Discussion and
conclusion: Helicobacter pylori infection has a very high prevalence in the age group of children
in Sanandaj, and this prevalence increases with age. Contamination starts from early childhood,
even before the start of complementary feeding, and it seems that transmission by other means
plays an important role, along with transmission by food and water. Although milk has a protective
effect, it cannot prevent this infection.
