Abstract:
اهداف: در مراقبت های سالمندی که در مراکز سلامت اجرا می شود، سالمندان سالم مورد غفلت مراقبتی و آموزشی واقع می گردند. این مطالعه کیفی با هدف استاندارد سازی مراقبت سالمندی سالم به انجام رسید.
مواد و روشها: این تحقیق عملکردی مشارکتی از 1395 تا 1397 با مشارکت 11 مراقب سلامت، 54 سالمند و 54 عضو خانواده در مرکز بهداشت مطهری مشهد، به انجام رسید. میانگین سنی مراقبین سلامت 2/7 ± 32/4 سال و میانگین سابقه کاری 8/3 سال بود. میانگین سنی سالمندان، 3/9 ± 64/3 سال بود. ابزار گردآوری دادهها پرسشنامه سروکووال، مصاحبه، بحث گروهی متمرکز و یادداشت برداری عرصه بود. پس از شناسایی مشکلات از طریق مصاحبه با 8 نفر از پرسنل و 19 سالمند، جلسات بحثهای گروهی جهت طراحی برنامه برگزار شد پس از اجرای برنامههای تغییر مجددا 30 مصاحبه با مشارکت کنندگان انجام و مقایسه قبل و بعد از برنامههای تغییر انجام وارزشیابی شدند. برای تحلیل محتوی کیفی از روش توصیف واقعیت و جهت تحلیل دادههای کمی از آمار توصیفی و آزمون آماری تی زوجی استفاده شد.
یافتهها: برآیند مطالعه حاضر مدل مراقبتی پویا بود که توانست با اصلاح نگرش، مهارتها و توانمندسازی پرسنل سبب بهبود مراقبت های سالمندی سالم در محیط مرکز بهداشت شود. میانگین نمره کلی کیفیت خدمات از نظر سالمندان و خانوادهها در مرحله قبل از مداخله به ترتیب، 9/46 ± 63/02 و 9/05 ± 61/83 بود. این مقادیر در مرحله بعد از مداخله به 14/75 ± 130/09 و 13/56 ± 122/65 رسید که از نظر آماری، تفاوت معنیدار بود (0/001P = ).
نتیجهگیری: مدل پویا با ویژگیهایی همچون مراقبت فعال، مشارکتی، مستمر، جامع و حمایتی سبب ارتقاء کیفیت خدمات سالمندی شد. بنابراین این مدل میتواند در سایر مراکز خدمات سلامت بکار رود.
Objectives: In elderly care provided by health centers, healthy elderly are neglected in care and education. This qualitative study aimed to standardize healthy ageing care.
Methods & Materials: This participatory action research was conducted from 1395 to 1397 in Motahari Health Center of Mashhad, with the participation of 11 health care provider, 54 healthy elderly and 54 family members. The mean age of caregivers was 32.4 ±2.7 years and their mean work experience was 8.3 years. The mean age of the elderly was 64.3 ± 3.9 years. Data were collected through SERVQUAL questionnaire, interviews, focus group discussions, and field notes. After identifying problems through interviewing 8 staff and 19 seniors, Group discussion sessions were held to design the program. After implementing the change plans, 30 interviews with the participants were conducted again and the comparisons were done and evaluated before and after the change plans. For qualitative content analysis, the method of describing reality was used, and for analysing quantitative data, descriptive statistics and paired t-test were used.
Results: The result of this study was dynamic care model could be improve healthy aging care in the health center environment by modifying attitudes, skills and staff empowerment. The mean score of quality of care for the elderly and their families in the pre-intervention phase was 63.02 ± 9.46 and 61.83 ± 9.05, respectively. In the post-intervention phase, these values reached 130.19 ± 14.75 and 122.65 ± 13.56, respectively, which were statistically significant (P=0.001).
Conclusions: The dynamic model promoted the quality of elderly services with features such as active, participatory, continuous, comprehensive and supportive care. So this model can be used in other health
